      <description>&lt;p&gt;Molecular dynamics (MD) are extremely complex, yet understanding the slow components of their dynamics is essential to understanding their macroscopic properties. To achieve this, one models the MD as a stochastic process and analyses the dominant eigenfunctions of the associated Fokker-Planck operator, or of closely related transfer operators. So far, the calculation of the discretized operators requires extensive MD simulations. The square-root approximation of the Fokker-Planck equation is a method to calculate transition rates as a ratio of the Boltzmann densities of neighboring grid cells times a flux, and can in principle be calculated without a simulation. In a previous work we still used MD simulations to determine the flux. Here, we propose several methods to calculate the exact or approximate flux for various grid types, and thus estimate the rate matrix without a simulation. Using model potentials we test computational efficiency of the methods, and the accuracy with which they reproduce the dominant eigenfunctions and eigenvalues. For these model potentials, rate matrices with up to [Formula see text] states can be obtained within seconds on a single high-performance compute server if regular grids are used.Ba0.85Ca0.15Ti0.9Zr0.1O3(BCZTO) ferroelectric ceramic loaded with Ag nanoparticles (NPs) was explored for their photo/piezocatalytic performance. The presence of Ag loading on BCZTO ceramic was confirmed using the electron microscopes. X-ray photoelectron spectroscopy revealed the metallic chemical state of Ag NPs loaded on the surface of BCZTO ceramic. The absorbance spectrum of the Ag loaded BCZTO sample showed visible light absorption hump due to the phenomenon of surface plasmonic resonance (SPR). During the photocatalysis process, the ~99% of rhodamine B (RB) dye was degraded in aqueous solution using Ag loaded BCZTO sample showing its promising photocatalysis activity. This phenomenon is observed both in stress-constrained geometry optimizations and in ab initio molecular dynamics simulations at finite temperature and applied stress.The mechanical properties of Au nanoparticle arrays are studied by tensile and compressive deformation, using large-scale molecular dynamics simulations which include up to 16 million atoms. Our results show that mechanical response is dominated by nanoparticle size. For compression, strength versus particle size shows similar trends in strength than full-density nanocrystals. For diameters (d) below 10 nm there is an inverse Hall-Petch (HP) regime. Beyond a maximum at 10 nm, strength decreases following a HP d -1/2 dependence. In both regimes, interparticle sliding and dislocation activity play a role. The array with 10 nm nanoparticles showed the same mechanical properties than a polycrystalline bulk with the same grain size. This enhanced strength, for a material nearly 20% lighter, is attributed to the absence of grain boundary junctions, and to the array geometry, which leads to constant flow stress by means of densification, nanoparticle rotation, and dislocation activity. For tension, there is something akin to brittle fracture for large grain sizes, with NPs debonding perpendicular to the traction direction. The Johnson-Kendall-Roberts contact theory was successfully applied to describe the superlattice porosity, predicting also the array strength within 10% of molecular dynamics values. Although this study is focused on Au nanoparticles, our findings could be helpful in future studies of similar arrays with NPs of different kinds of materials.A magnetic resonance (MR) shear wave elastography technique that uses transient acoustic radiation force impulses from a focused ultrasound transducer and a sinusoidal-shaped MR displacement encoding strategy is presented. Using this encoding strategy, an analytic expression for calculating the shear wave speed in a heterogeneous medium was derived. Green's function-based simulations were used to evaluate the feasibility of calculating shear wave speed maps using the analytic expression. Accuracy of simulation technique was confirmed experimentally in a homogeneous gelatin phantom. The elastography measurement was compared to harmonic magnetic resonance elastography in a homogeneous phantom experiment and the measured shear wave speed values differed by less than 14%. This new transient elastography approach was able to map the position and shape of inclusions sized from 8.5 to 14 mm in an inclusion phantom experiment. We report a case of a short radiolunate ligament avulsion injury and a volar wrist capsular injury that went unrecognized for an extended period and led to chronic wrist pain and instability.  Normal digital flexion relies on flexor tendon pulleys to convert linear muscular force to angular digital motion. However, there is a growing trend to vent them partially during flexor tendon repair. The objective of this study was to examine the effects of a thermoplastic ring, acting as an external pulley, on flexor tendon biomechanics and finger range of motion (ROM) after pulley venting. &lt;/p&gt;

&lt;p&gt;We tested 15 cadaveric digits using an invitro active finger motion simulator. We measured loads induced by flexor digitorum superficialis (FDS) and flexor digitorum profundus (FDP) as well as joint ROM with sequential sectioning of the A2, A3, and A4 pulleys compared with an intact pulley condition. At each stage, external thermoplastic pulley rings were applied snugly over the proximal and middle phalanges to recreate A2 and A4 function, respectively. &lt;/p&gt;

A two-substrate model was applied to describe the evident existence of rapid and slowly degradable material. Regarding the hydrolysis kinetic constants predicted by this model, considerable increases in the rapid biodegradation stage (krapid) were observed when comparing to the values found for the slow (kslow) biodegradation stage in all the cases tested. In this study, a facile and versatile approach was adopted to synthesize a series of novel fluorinated hematoporphyrin ether derivatives (I1-I5 and II1-II4), and the photodynamic activities of these compounds were studied. Compared to hematoporphyrin monomethyl ether (HMME), all PSs showed preferable photodynamic activity against A549 lung tumor cells. The longest visible absorption wavelength of these compounds was approximately 622 nm. To our knowledge, this is the first study indicating genistein, 8-prenylgenistein, apigenin, 8-prenylapigenin, 8-prenylnaringenin, galangin and valoneic acid dilactone as natural molecules with high inhibitory potency toward GP. We identified i) the residues Phe285, Tyr613, Glu382 and/or Arg770 as the most relevant for the binding of the best flavonoids to the inhibitor site of GP, and ii) the 5-OH, 7-OH, 8-prenyl substitutions in ring A and the 4'-OH insertion in ring B to favor flavonoid binding at this site. Our results are invaluable to plan further structural modifications through organic synthesis approaches and develop more effective pharmaceuticals for Type 2 Diabetes treatment, and serve as the starting point for the exploration of food products for therapeutic usage, as well as for the development of novel bio-functional food and dietary supplements/herbal medicines. &lt;br&gt;

Though deforestation, habitat destruction, natural calamities, illegal hunting, and climate changes are the challenging threats for bats, the role of existing gastrointestinal (GI) parasites have not been evaluated yet in Nepal. Thus, the current study aims to determine the prevalence of GI parasites in bats from the Shaktikhor area at the Chitwan district of Southcentral Nepal. From July 2018 to February 2019, a total of 60 fecal samples of bats (30 from frugivorous bats and 30 from the insectivorous bats) were collected. These samples were preserved at 2.5% potassium dichromate solution. The fecal examination was carried out by the direct wet mount, concentrations, acid-fast staining, and sporulation techniques. Overall results showed the prevalence rate of 80% GI parasites. The parasites detected in the insectivorous bats were Ascarid spp., Capillarid sp., Cryptosporidium sp., Eimeria spp., Entamoeba sp., Giardia sp., Hymenolepis spp., Isospora sp., Oxyurid sp., Strongyle, and Strongyloides sp. In contrast, Eimeria sp., Entamoeba sp., and Hymenolepis sp. were detected in the frugivorous bats. Based on a wide diversity of parasite richness and parasitic concurrency measured by the prevalence rates, we suggest that GI parasitism might be a threatening factor in the insectivorous bats in the current study area.Right atrial diverticulum is a very rare anomaly. It is an outpouching arising from the right atrial free wall. Clinical presentations vary widely but some cases are associated with supraventricular tachycardia and atrial flutter/fibrillation. The incidence/prevalence of this anomaly is not available because only a few cases have been reported. We report a 38-year-old female patient who presented to the Heart Clinic, Tehran, Iran in 2019 with a history of dyspnea and chest pain. Electrocardiography revealed left bundle branch block. Following a magnetic resonance imaging study, the patient was diagnosed with a right atrial diverticulum. She underwent surgical resection of the diverticulum. The post-operative course was uneventful and no recurrence of the arrhythmia was detected during the six months of follow-up. To the best of the authors' knowledge, this combination has not been described in the literature.Managing a high intracoronary (IC) thrombus burden is a major challenge in patients with ectatic coronary arteries who present with an ST-elevation myocardial infarction (STEMI). We report a 45-year-old male patient who presented to the Department of Medicine, Sultan Qaboos University Hospital, Muscat, Oman, in 2017 with an inferior STEMI. Coronary angiography revealed ectatic coronary arteries with an occluded right coronary artery (RCA). The RCA had a massive thrombus burden that did not resolve with aspiration thrombectomy or balloon angioplasty. find more Subsequently, IC thrombolysis was administered resulting in significant reperfusion. During the course of his treatment, the patient suffered from intracranial bleeding as a complication; however, he recovered completely with no residual neurological sequelae. This case report demonstrates that IC thrombolysis is a valid option in patients with a massive thrombus burden in the setting of a STEMI, albeit with an increased risk of major bleeding.Pierson syndrome is caused by mutations in the laminin β2 gene causing absent β2 laminin, which is a normal component of the basement membranes of the mature glomerulus, structures in the anterior eye and neuromuscular junctions. The mutations manifest as congenital nephrotic syndrome and microcoria which are characteristic ocular features of this disease. These mutations may also result in neurological abnormalities such as hypotonia and psychomotor retardation. We report a two-month old boy who presented to the Pediatrics Department of Dr. Ruth K. M. Pfau Civil Hospital, Karachi, Pakistan, in 2015, with the typical features of microcoria and congenital nephrotic syndrome. The hypocalcaemia, hypoproteinaemia and probable immunocompromised state consequent to nephrotic syndrome resulted in seizures, hypothyroidism and urosepsis. Despite being treated aggressively with high dose antibiotics, ionotropic support, angiotensin-converting enzyme inhibitors, thyroxine replacement and nutritional support, the infant died due to significant multiorgan disease including renal failure and septic shock. &lt;br&gt;
 The aim of this study was to determine the factors that affect the provision of bystander cardiopulmonary resuscitation (CPR) among the general public in Oman. &lt;/p&gt;

&lt;p&gt;This was a descriptive cross-sectional study performed over a two-month period from November to December 2018 using a questionnaire among the general public with no medical background. &lt;/p&gt;

&lt;p&gt;A total of 1,080 individuals were included in the study. The median age of the participants was 27 (20-39) years old with 624 (57.8%) male participants. More than half of the participants (52.9%) were willing to perform CPR on a stranger. Lack of knowledge of CPR and a fear that they might cause more harm were the main reasons precluding people from performing CPR. Age, educational status and having attended a CPR course were predictors of willingness to perform CPR. &lt;/p&gt;

&lt;p&gt;There is a positive attitude towards bystander CPR in Oman among the general public. More education and training opportunities should be made available in workplaces and educational institutions. &lt;br&gt;

      <description>&lt;p&gt;Nanobubbles have attracted great interest in recent times because of their application in water treatment, surface cleaning, and targeted drug delivery, yet the challenge remains to gain thorough understanding of their unique behavior and dynamics for their utilization in numerous potential applications. In this work, we have used a liquid-phase electron microscopy technique to gain insights into the quasistatic merging of surface nanobubbles. The electron beam environment was controlled in order to suppress any new nucleation and slow down the merging process. Ivacaftor The transmission electron microscopy study reveals that merging of closely positioned surface nanobubbles is initiated by gradual localized changes in the physical properties of the region between the adjoining nanobubble boundary. The observed phenomenon is then analyzed and discussed based on the different perceptions localized liquid density gradient and bridge formation for gas exchange. In this study, it is estimated that the merging of the stable nanobubbles is initiated by the formation of a thin gas layer. This work not only enhances our understanding of the merging process of stable surface nanobubbles but will also lead to exploration of new domains for nanobubble applications.Various approaches for transmitting ions from atmosphere to the deep vacuum required for mass analysis have been developed with the goal to increase the ion to gas ratio while maintaining high ion transmission efficiency. Since the vast majority of ion losses occurs in the atmospheric pressure ion source, an effective way to improve sampling of those ions is to increase the atmosphere to vacuum aperture diameter. However, as the aperture diameter is increased, the resulting intense free jet gas expansion and subsequent gas beam can scatter ions in the first vacuum region. The interface described here provides an optimized flow field to the second vacuum stage, with a unique geometry to counter the ion losses from scattering collisions with the gas. Two additional differentially pumped quadrupole ion guides are used to further improve the ion to gas ratio, resulting in an ion transfer efficiency improvement of 5-6× over a two-stage differentially pumped interface with quadrupole ion guides. The interface also demonstrates efficient declustering and fragmentation capabilities beneficial for reducing background chemical noise.Polymers that are biocompatible and degradable are desired for tissue engineering approaches in the treatment of vascular diseases, especially for those involving small-diameter blood vessels. Herein, we report the compatibility of a newly developed glycerol-based aliphatic polycarbonate possessing simple methoxy side groups, named poly(5-methoxy-1,3-dioxan-2-one) (PMDO), with blood cells and plasma proteins as well as its susceptibility to hydrolysis. As a consequence of the organocatalytic ring-opening polymerization (ROP) of a methoxy-functionalized cyclic carbonate derived from glycerol, PMDO with a sufficiently high molecular weight (Mn 14 kg/mol) and a narrow distribution (D̵M 1.12) was obtained for evaluation as a bulk biomaterial. This study demonstrates for the first time the organocatalytic ROP of a glycerol-based cyclic carbonate in a controlled manner. Compared with the clinically applied aliphatic polycarbonate poly(trimethylene carbonate) (PTMC), PMDO inhibits platelet adhesion by 33% and denaturation of fibrinogen by 23%. Although the wettability of PMDO based on water contact angle was almost comparable to those of PTMC and poly(ethylene terephthalate), the reason for the inhibited platelet adhesion and protein denaturation appeared to be related to the presence of specific hydrated water formed in the hydrated polymer. The improved hydration of PMDO also enhanced the susceptibility to hydrolysis, with PMDO demonstrating a slightly higher hydrolytic property than PTMC. This simple glycerol-based aliphatic polycarbonate has the following benefits bio-based characteristics of glycerol and improved blood compatibility and hydrolytic biodegradability stemming from moderate hydration of the methoxy side groups.Photophysical properties of europium (Eu(III)) complexes are affected by ligand-to-metal charge transfer (LMCT) states. Two luminescent Eu(III) complexes with three tetramethylheptadionates (tmh) and pyridine (py), &lt;a href="https://dev.toseven-coordinated%20monocapped-octahedral%20structure"&gt;Eu(tmh)3(py)1&lt;/a&gt; and &lt;a href="https://dev.toeight-coordinated%20square%20antiprismatic%20structure"&gt;Eu(tmh)3(py)2&lt;/a&gt;, were synthesized for geometrical-induced LMCT level control. Distances between Eu(III) and oxygen atoms of tmh ligands were estimated using single-crystal X-ray analyses. The contribution percentages of π-4f mixing in HOMO and LUMO at the optimized structure in the ground state were calculated using DFT (LC-BLYP). The Eu-O distances and their π-4f mixed orbitals affect the energy level of LMCT states in Eu(III) complexes. The LMCT energy level of an eight-coordinated Eu(III) complex was higher than that of a seven-coordinated Eu(III) complex. The energy transfer processes between LMCT and Eu(III) ion were investigated using temperature-dependent and time-resolved emission lifetime measurements of 5D0 → 7F J transitions of Eu(III) ions. In this study, the LMCT-dependent energy transfer processes of seven- and eight-coordinated Eu(III) complexes are demonstrated for the first time.A variety of ene-yne cross metathesis reactions were performed using unsaturated phosphonate and phosphate reagents, affording the corresponding phosphorylated 1,3-diene products in good to excellent yields. These difficult ene-yne metatheses employed a Grubbs catalyst bearing a cyclic amino alkyl carbene ligand. A variety of terminal alkynes of varying substitution underwent the reaction, and different phosphorus-containing alkenes were found to give the conjugated diene products in high yields. The resulting dienes were further transformed by Horner-type Wittig reactions and a Diels-Alder cycloaddition.Very-short- (vSCCPs, C6-9), short- (SCCPs, C10-13), medium- (MCCPs, C14-17), and long-chain chlorinated paraffins (LCCPs, C&amp;gt;17) were analyzed in indoor air and dust collected from the living rooms and personal 24 h air of 61 adults from a Norwegian cohort. Relatively volatile CPs, i.e., vSCCPs and SCCPs, showed a greater tendency to partition from settled indoor dust to paired stationary indoor air from the same living rooms than MCCPs and LCCPs, with median logarithmic dust-air partition ratios of 1.3, 2.9, 4.1, and 5.4, respectively. Using the stationary indoor air and settled indoor dust concentrations, the combined median daily exposures to vSCCPs, SCCPs, MCCPs, and LCCPs were estimated to be 0.074, 2.7, 0.93, and 0.095 ng/kg bw/d, respectively. Inhalation was the predominant exposure pathway for vSCCPs (median 99%) and SCCPs (59%), while dust ingestion was the predominant exposure pathway for MCCPs (75%) and LCCPs (95%).       <description>&lt;p&gt;A high-sensitivity sensor to measure titanium atom density based on time-resolved cavity ring-down spectroscopy (CRDS) was developed to monitor the wall erosion and predict the lifetime of Hall thrusters. The minimum detection limit for the sensor was dependent on the discharge current oscillation in the Hall thruster. A Volterra engine management system was employed for time-resolved measurements to develop the time-resolved CRDS system, which was synchronized to the discharge current oscillation. The results confirmed that the path-integrated number density of sputtered titanium atoms was synchronized with the discharge current oscillation. The minimum detection limit was decreased by ∼30% from 2 × 1012 to 6 × 1011 m-2.We report an experimental protocol for measuring the frequency dependence of the bulk modulus carried out in a synchrotron x-ray facility based multi-anvil high-pressure apparatus. An oscillating pressure perturbation characterized by x-ray diffraction produces a volume strain measured by imaging. Together, these yield the bulk modulus of the sample. Here, we report data at 3 mHz as an example of the possibility of providing these data for the frequency range of 1 mHz-100 mHz.Two high-temperature 2D-pyrometers are described in this work. They are, respectively, based on the comparison of light intensities received by two monochrome cameras with narrow filters and the different color channels in a single color camera. The calibration procedure includes tests with thermocouples in flame flue gases and with a light bulb filament in the range 1050 K-2650 K, although the upper limit may be reliably extrapolated in the two-camera pyrometer. This device shows smaller uncertainties in absolute temperature determination than the other, ∼±15 K in the calibration curve. A combination of the two-color technique with intensity pyrometry is shown to allow the quantification of differences of the order of 1 K for uniform emissivity surfaces. Several examples of application of both pyrometers are presented to assess their spatial resolution and thermal capabilities, including the contact between the hot filament and its support in the light bulb and fine-wire thermocouples with large bead/wire ratios. The optical systems were designed for their use at short working distances and with high spatial resolution (4.7 μm/pixel and 8.7 µm/pixel) but could be adapted to other scenarios.In neutron scattering on soft matter, an important concern is the control and stability of environmental conditions surrounding the sample. Complex sample environment setups are often expensive to fabricate or simply not achievable by conventional workshop manufacturing. We make use of state-of-the-art 3D metal-printing technology to realize a sample environment for large sample sizes, optimized for investigations on thin film samples with neutron reflectometry (NR) and grazing-incidence small-angle neutron scattering (GISANS). Selleckchem Vorinostat With the flexibility and freedom of design given by 3D metal-printing, a spherical chamber with fluidic channels inside its walls is printed from an AlSi10Mg powder via selective laser melting (SLM). In order to optimize the channel layout, flow simulations were carried out and verified in temperature switching tests. Exemplary GISANS and NR measurements in time-of-flight mode are shown to prove that the chamber provides a stable, background free sample environment for the investigation of thin films.We report the characteristics of extracted beam current in the test plasma produced by direct current for sheet plasma upgrade producing negative ions by volume production without cesium (Cs) seeding. The negative hydrogen ion beam is extracted by a two-grid extraction system, which is located at the periphery of the sheet plasma. Once linearized, the dynamics of the constant-frequency closed-loop system are reduced to a single transfer function. By modifying the bandwidth of this transfer function, a tradeoff is achieved between image noise and imaging speed. In this article, a new constant-frequency feedback loop is developed, utilizing the self-excitation technique for resonating the cantilever. Along with the proposed controller, it will be shown with the root locus that one needs to vary a single parameter, the loop gain, to modify the closed-loop bandwidth. The result is a robust, low-order, real-poled, feedback loop that is very easy to tune. Our aim was to measure the prevalence of burnout and distress among physicians in a cardiovascular centre of a quaternary hospital network in Canada, and compare these outcomes to those for physicians at academic health science centres (AHSCs) in the United States. &lt;/p&gt;

&lt;p&gt;We conducted a survey of physicians practising in a cardiovascular centre at 2 quaternary referral hospitals in Toronto, Ontario, between Nov. 27, 2018, and Jan. 31, 2019. The survey tool included the Well-Being Index (WBI), which measures fatigue, depression, burnout, anxiety or stress, mental and physical quality of life, work-life integration, meaning in work and distress; a score of 3 or higher indicated high distress. We also evaluated physicians' perception of the adequacy of staffing levels and of fair treatment in the workplace, and satisfaction with the electronic health record. We carried out standard univariate statistical comparisons using the re (2.4 v. 1.8, &lt;br&gt;
 = 0.004) and reported a higher prevalence of burnout (65.4% v. 56.6%, &lt;br&gt;

We describe and compare testing for SARS-CoV-2, test positivity and hospital admission, receipt of intensive care and mortality rates related to COVID-19 for people with a recent history of homelessness versus community-dwelling people as of July 31, 2020. &lt;/p&gt;

&lt;p&gt;We conducted a population-based retrospective cohort study in Ontario, Canada, between Jan. 23 and July 31, 2020, using linked health administrative data among people who either had a recent history of homelessness or were dwelling in the community. People were included if they were eligible for provincial health care coverage and not living in an institutionalized facility on Jan. 23, 2020.  Surgeons frequently care for children who have sustained gunshot wounds (GSWs). However, firearm safety education is not a focus in general surgery training. We hypothesised that firearm safety discussions do not routinely take place when children present to a trauma centre with a GSW. &lt;/p&gt;

&lt;p&gt;A retrospective review of patients &amp;lt;18 years presenting with GSWs to a level 1 paediatric trauma centre from 2009 to 2019 was performed. &lt;p&gt;A total of 226 patients with GSWs were identified, 22% were unintentional and 63% were assault. Firearm safety discussions took place in 10 cases (4.4%). Firearm safety discussions were more likely to occur after unintentional injuries compared with other mechanisms (16.0% vs 1.3%, p&amp;lt;0.001). CPS was contacted in 29 cases (13%). CPS notification was more likely for unintentional injuries compared with other mechanisms (40% vs 3.9%, p&amp;lt;0.001) and for younger patients (7 years vs 15 years, p&amp;lt;0.001). &lt;/p&gt;

Context-driven implementation strategies are critical for successful implementation. However, there is no practical, evidence-based guidance on how to map the context in order to design context-driven strategies. Therefore, this practice paper describes the development and validation of a systematic context-mapping tool. The tool was cocreated with local end-users through a multistage approach. As proof of concept, the tool was used to map beliefs and behaviour related to chronic respiratory disease within the FRESH AIR project in Uganda, Kyrgyzstan, Vietnam and Greece. Feasibility and acceptability were evaluated using the modified Conceptual Framework for Implementation Fidelity. Effectiveness was assessed by the degree to which context-driven adjustments were made to implementation strategies of FRESH AIR health interventions.
