The approach can be used for patient phenotyping, feature extraction, or, as in our motivating example, outcome identification, which refers to the process of identifying disease status through patient laboratory tests rather than through diagnosis codes or prescription information. We model the joint distribution of a continuous longitudinal outcome and baseline covariates using an enriched Dirichlet process prior. This joint model decomposes into (local) semiparametric linear mixed models for the outcome given the covariates and simple (local) marginals for the covariates. The nonparametric enriched Dirichlet process prior is placed on the regression and spline coefficients, the error variance, and the parameters governing the predictor space. This leads to clustering of patients based on their outcomes and covariates. We predict the outcome at unobserved time points for subjects with data at other time points as well as for new subjects with only baseline covariates. We find improved prediction over mixed models with Dirichlet process priors when there are a large number of covariates. Our method is demonstrated with electronic health records consisting of initiators of second-generation antipsychotic medications, which are known to increase the risk of diabetes. We use our model to predict laboratory values indicative of diabetes for each individual and assess incidence of suspected diabetes from the predicted dataset. &lt;br&gt;

 Treatment of immune thrombocytopenia (ITP) usually involves long-term use of immunosuppressive corticosteroids and splenectomy. However, these treatments often have side effects in patients. The Mongolian medicine Qishunbaolier (QSBLE) has a high curative effect, reduces the chances of relapse, and has no obvious side effects. This study was designed to identify potential therapeutic targets of QSBLE for treating ITP. &lt;/p&gt;

&lt;p&gt;To reveal differences in protein expression between ITP patients (ITPs) before and after QSBLE treatment, comparative proteomics studies were performed using isobaric tags for relative and absolute quantification (iTRAQ). The analysis used nanospray liquid chromatography/tandem mass spectrometry (nano-LC/MS/MS) in positive ion electrospray ionization mode. Key proteins relevant to ITP were revealed by the Kyoto Encyclopedia of Genes and Genomes (KEGG) and other bioinformatics tools. Real-time polymerase chain reaction (RT-PCR) analysis was carried out for confirmation of differentially expreor QSBLE for ITP therapy and biomarkers.This study investigated kimchi-induced apoptosis in HT-29 human colon carcinoma cells. Three types of kimchi samples were prepared standardized kimchi brined with general commercial Baechu cabbage by a standardized recipe (SK), Amtak Baechu kimchi brined with Amtak Baechu cabbage by a standardized recipe (AmK), and anticancer kimchi brined with organically cultivated Baechu cabbage by a functional recipe (AK). MTT assay, qRT-PCR, and Western blotting analysis were performed. The results indicate that AmK and AK, especially AK significantly upregulated mRNA expression of apoptosis-related genes Bim, Bax, Bak, caspase-8, -9, -3, and p53 but suppressed Bcl-xL and Bcl-2 expression. In addition, AK treatment significantly upregulated protein expression levels of caspase-3 but strikingly reduced the protein expression level of Bcl-2 (p less then .05), followed by AmK treatment. Our data suggest that AK and AmK can markedly suppress the proliferation of HT-29 cells via activation of apoptosis. In this study, an electrochemical cTn1 biosensor, MoS2 nanoflowers on screen-printed electrodes assisted by aptamer, was synthesized using hydrothermal technique. Field emission scanning electron microscopy revealed distinct 2D nanosheets and jagged flower-like 3D MoS2 nanoflower structure, with X-ray diffraction analysis revealing well-stacked MoS2 layers. Voltammetry aptasensing of cTn1 ranges from 10 fM to 1 nM, with a detection limit at 10 fM and a sensitivity of 0.10 nA µM-1 cm-2 . This is a ∼fivefold improvement in selectivity compared with the other proteins and human serum. ANS binding sites in the protein film are specific and could be saturated by ANS titration. ANS located in the binding site that exhibits the long-lifetime fluorescence is not accessible to the water molecules and its DAS stays homogeneously broadened upon hydration of the protein film. In contrast, ANS from the sites demonstrating the short-lifetime fluorescence is accessible to water molecules. In the hydrated films, solvent-induced fluctuations produce an ensemble of binding sites with similar characters. Therefore, upon hydration, the short-lifetime DAS becomes significantly red-shifted and inhomogeneously broadened. The similar spectral features have previously been observed for ANS complexed with globular proteins in solution. The data reveal the origin of the short-lifetime fluorescence component of ANS bound to the globular proteins in aqueous solution.       <description>&lt;p&gt;Three-dimensional (3D) printing technology is serving as a promising approach of fabricating titanium (Ti) and its alloys used for bone tissue engineering. find more However, the biological inertness nature of Ti material limits its capability to bind directly with the bone tissue. This paper aims to enhance the bioactivity and osteogenesis of 3D printed Ti-6Al-4V implants by constructing a hierarchical micro/nano-topography on the surface. Ti-6Al-4V implants were prepared by the electron beam melting (EBM) technique. A method combining ultrasonic acid etching with anodic oxidation is proposed for surface modification of EBM Ti-6Al-4V implants in this study. The acid etching step was to remove any existent residual powders on the implant's surface and construct micro-pits and -grooves on the EBM microrough surface. Nanotube arrays with a diameter of 40-50 nm were superimposed on the micro-structured substrate via anodic oxidation. The results of in vitro experiments showed that the hierarchical micro/nano-structured suIFICANCE Traditional titanium implants have the nature of biological inertness, which limits their capability to bind directly with the bone tissue. The failure of implants after couple of years of implantation will cause huge pain to the patients. In this work, a surface modification method for 3D printed implants was developed to construct a hierarchical micro/nano-structure. Through the in vitro and in vivo experiments, we proved that this hierarchical micro/nano-structure induced a better promotion effect on osteoblast proliferation and differentiation comparing with untreated surface or polished surface, and was also capable of bolstering the new bone formation, suggesting a potent strategy to improve the biological properties of 3D printed titanium implants. The work is expected to accelerate the application of 3D printed orthopedic and dental implants in clinics.Motivated by the need for self-disinfecting materials that can be used to reduce the surface transmission of harmful microbes to healthy hosts, here we prepared a photodynamic antimicrobial membrane comprised of electrospun cellulose diacetate (CA) microfibers into which the photosensitizer protoporphyrin IX (PpIX) was in situ embedded. The resultant porous PpIX-embedded CA (PpIX/CA) microfibrous membranes were prepared with two different photosensitizer loadings 5 and 10 wt% PpIX with respect to CA (85 and 170 nmol PpIX/mg membrane, respectively). The singlet oxygen (1O2) generated by the embedded photosensitizer was confirmed by electron paramagnetic resonance spectroscopic studies through generation of the TEMPO radical, and its photooxidation efficiency was further investigated using potassium iodide as a model substrate. Antibacterial photodynamic inactivation studies showed that the PpIX/CA membrane achieved a 99.8% reduction in Gram-positive S. aureus after illumination (Xe lamp, 65 ± 5 mW/cm2, λ ≥ 420 nm; 30 min), with a lower level of reduction (86.6%) for Gram-negative E. coli. Potentiation with potassium iodide was found to be an effective way to further enhance the antimicrobial efficacy of the PpIX/CA microfibrous membrane, achieving 99.9999% (6 log units) inactivation of both S. aureus and E. coli in the presence of 25 and 100 mM KI, respectively. These findings indicate that the electrospun CA microfibrous membrane is an ideal matrix for a photosensitizer such as PpIX to be embedded and effectively sensitized upon visible light illumination, and its antimicrobial photodynamic inactivation efficiency could be strongly enhanced with the increased KI addition, showing a promising future for its use in pathogen transmission defensive materials.Two core-double-shell pH-sensitive nanocarriers were fabricated using Fe3O4 as magnetic core, poly(glycidyl methacrylate-PEG) and salep dialdehyde as the first and the second shell, and doxorubicin as the hydrophobic anticancer drug. Two nanocarriers were different in the drug loading steps. The interaction between the first and the second shell assumed to be pH-sensitive via acetal cross linkages. The structure of nanocarriers, organic shell loading, magnetic responsibility, morphology, size, dispersibility, and drug loading content were investigated by IR, NMR, TG, VSM, XRD, DLS, HRTEM and UV-Vis analyses. The long-term drug release profiles of both nanocarriers showed that the drug loading before cross-linking between the first and second shell led to a more pH-sensitive nanocarrier exhibiting higher control on DOX release. Cellular toxicity assay (MTT) showed that DOX-free nanocarrier is biocompatible having cell viability greater than 80% for HEK-293 and MCF-7 cell lines. Besides, high cytotoxic effect observed for drug-loaded nanocarrier on MCF-7 cancer cells. Cellular uptake analysis showed that the nanocarrier is able to transport DOX into the cytoplasm and perinuclear regions of MCF-7 cells. In vitro hemolysis and coagulation assays demonstrated high blood compatibility of nanocarrier. The results also suggested that low concentration of nanocarrier have a great potential as a contrast agent in magnetic resonance imaging (MRI).Selective delivery of drugs to damaged tissues favorable to reduce the side effects while enhancing the therapeutic efficacy. Molecular correction was performed using seven microsatellite markers. Samples from treatment failures were genotyped for the pfk13, pfcrt, and pfmdr1 genes. Day 3 clearance rates were ≥95% in all arms. Uncorrected day 28 Kaplan-Meier efficacy estimates ranged from 84.2 to 90.1% for the AL arms and 84.7 to 100% for the ASAQ arms. Corrected day 28 estimates were 87.6% (95% confidence interval [CI], 81 to 95%) for the AL arm in Lunda Sul, 92.2% (95% CI, 87 to 98%) for AL in Zaire, 95.6% (95% CI, 91 to 100%) for ASAQ in Zaire, 98.4% (95% CI, 96 to 100%) for AL in Benguela, and 100% for ASAQ in Benguela and Lunda Sul. Thus, we investigated the in vitro activity of cefiderocol against a total of 246 clinical isolates of Burkholderia pseudomallei from Queensland, Australia. The collection was composed primarily of bloodstream (56.1%), skin and soft tissue (16.3%), and respiratory (15.9%) isolates. MICs of cefiderocol ranged from ≤0.03 to 16 mg/liter, whereas the MIC90 was 0.125 mg/liter. Based upon CLSI clinical breakpoints for cefiderocol against Pseudomonas aeruginosa, Acinetobacter baumannii, and Stenotrophomonas maltophilia, three isolates (1.2%) would be classified as nonsusceptible (MIC &amp;gt; 4 mg/liter). However, combined treatment with tacrolimus and low-dose steroid has not been compared with high-dose steroid for induction of clinical remission in a large-scale randomized study. &lt;/p&gt;

&lt;p&gt;In this 24-week open-label noninferiority study, we randomized 144 adults with minimal change nephrotic syndrome to receive 0.05 mg/kg twice-daily tacrolimus plus once-daily 0.5 mg/kg prednisolone, or once-daily 1 mg/kg prednisolone alone, for up to 8 weeks or until achieving complete remission. Two weeks after complete remission, we tapered the steroid to a maintenance dose of 5-7.5 mg/d in both groups until 24 weeks after study drug initiation.       <description>&lt;p&gt;Chronic pain is a significant public health problem with emotional and disabling factors, which may not completely respond to current medical treatments such as opioids. The systematic review and meta-analysis aimed to examine the effectiveness and safety of MBCT for patients with chronic pain. Database searches of PubMed, Medline, EMBASE, the Cochrane Library, PsycINFO, Web of Science, Scopus and CINAHL up to 15 October 2019. Included studies assessed with the Cochrane risk-of-bias tool. Eight RCTs involved 433 patients, including chronic low back pain, fibromyalgia, migraine, rheumatoid arthritis and mix etiology. MBCT intervention demonstrated a short-term improvement on depression mood [standardized mean difference -0.72; 95% confidence interval = -1.22 to -0.22, p = 0.005] compared with usual care and was associated with short-term improvement in mindfulness compared with non-MBCT [SMD 0.51; 95% CI = 0.01 to 1.01, p = 0.04]. Between-group differences in pain intensity, pain inference and pain acceptance were not signiﬁcant at short- or long-term follow-up. Compared to active treatments, MBCT intervention not found significant differences in either short- or long-term outcomes. MBCT showed short-term efficacious on depressed mood and mindfulness of chronic pain patients. Longer follow-ups, large sample and rigorous RCTs that can be best understand remaining uncertainties needed. &lt;br&gt;

In this perspective, we highlight how proteomic studies have contributed to our understanding of autophagy in plants against the backdrop of yeast and animal studies.       <description>&lt;p&gt;Objective The outbreak of COVID-19 has different effects on people's psychological and social aspects. This study aimed to investigate the relationship between anxiety, self-compassion, and gender differences with self-care behaviors and fear of COVID-19 in Kermanshah. Method In the cross-sectional study, 403 people answered online questions on the Generalized Anxiety Disorder 7-Item (GAD-7) Scale, Self-Compassion Scale (SCS), Fear of COVID-19 Scale (FCV-19S), and a questionnaire focusing on COVID-19-related behavior. Pearson correlation coefficient and regression analysis were used for data analysis. Results There was a significant relationship between social distance and gender, and people who observed social distancing reported higher levels of fear of COVID-19 (p 0.05). There was a significant difference between those who answered yes to self-care behaviours related to washing and those who answered no in terms of variables of fear of COVID-19, the overall score of compassion, and subscales of compassion, including self-kindness and isolation (p less then 0.05). In relation to fear of COVID-19, married status, anxiety, and common humanity had a positive relationship with fear of COVID-19. However, self-judgment was negatively related to fear of COVID-19 (p less then 0.05). Conclusion According to the findings of the present study, men and people who are less afraid of COVID-19 are more likely not to observe self-care behaviors. Therefore, providing training about treatment protocols is necessary for these people. To reduce the fear of this disease in people with high levels of fear, psychologists, psychiatrists, and other medical staff can implement protocols to increase compassion.Objective COVID 19 is having a devastating effect on mankind's life. Individuals with COVID-19 will experience high levels of stress, depression, anxiety, and low quality of life. The goals the of present study were to investigate the effect of brief crisis intervention package on stress, depression, anxiety, and quality of life in patient with COVID-19. Method In this randomized controlled clinical trial, 30 patients were randomly selected and divided into control and experimental groups. The experimental group was examined in four 60-minute sessions for one month. During this period, the control group received only routine treatment and therapy. Before and after the intervention, DASS21, the Symptom Checklist (SCL-25), and WHO-QOL were used to measure and record patient mental health and quality of life. Finally, data were analyzed using SPSS 24 software. Results The average mental health score was assessed with WHO-QOL, DASS21, and SCL-25 before intervention and was not statistically significant (P = &amp;gt; 0.05), and the mean score of stress, anxiety, depression, WHO-QOL, SCL-25 after intervention was statistically significant (P  less then  0.001). Therefore, results showed the brief crisis intervention package was effective in improving the quality of life (P less then 0.05) and mental illnesses (P less then 0.05) in patients with COVID-19. Conclusion The brief crisis intervention package can reduce the negative effect of patients with COVID-19. Therefore, this intervention can be used as a beneficial treatment to improve mental disorder symptoms and to improve the condition of people who suffer from COVID-19.Objective We investigated the level of psychological problems, perceived risk, safety behaviors, and the potential roles of demographic variables, public trust, media, and prior anxiety to illness during the COVID-19pandemic among Iranians. Method In this cross sectional study, using a convenient and snowball sampling method, we distributed an online questionnaire to participants and collected data on their demographic variables, mental health status, the consumption of and level of trust to various media, the level of public trust, and perceived risk and safety behaviors regards COVID-19. Our final sample consisted of 1881 Iranian residents. We also found high percent of acute stress 52.71% (95% CI 50.45-54.96). Being female, married, and having a higher educational level increased the odds of safety behaviors. Public trust and national media can regulate the negative effect of the pandemic, while increasing perceived risk and appropriate safety behaviors can decrease psychological problems and disorders. However, social media increases perceived risk, safety behaviors, and psychological problems, especially severe acute stress. Conclusion We are in the early stages of the COVID-19 pandemic and its negative effects will increase. Many people have lost their relatives, their jobs, their social contacts, and are faced with a surge of negative news. Authorities should consider these critical issues and adopt appropriate communicative and supportive approaches to prevent their negative effects at both individual and societal levels.Objective Nowadays, the outbreak of Coronavirus (COVID-19) is one of the most stressful resources that has led to the rise of different levels of psychological crisis. Method This descriptive and correlational study was conducted on 354 adults living in Ardabil, Iran, who were selected using cluster sampling from 21 January to 19 March 2020. Self-Talk questionnaires, Coping Strategies, Death Anxiety, and Obsessive-Compulsive questionnaires were used for data collection. Descriptive statistics, Pearson correlation, and multiple linear regression were used for data analysis.
