      <title>Getting consumers and health care professionals within collaborative decisions in order to optimize care.</title>

We compared long-term frailty status, physical function, and quality of life among survivors of VTE with survivors of coronary heart disease (CHD) and stroke, and with those without these diseases. Methods and Results Cases of VTE, CHD, and stroke were continuously identified since ARIC (Atherosclerosis Risk in Communities Study) recruitment during 1987 to 1989. Functional measures were objectively captured at ARIC clinic visits 5 (2011-2013) and 6 (2016-2017); quality of life was self-reported. The 6161 participants at visit 5 were, on average, 75.7 (range, 66-90) years of age. By visit 5, 3.2% had had a VTE, 6.9% CHD, and 3.4% stroke. selleckchem Compared with those without any of these conditions, VTE survivors were more likely to be frail (odds ratio [OR], 3.11; 95% CI, 1.80-5.36) and have low ( less then 10) versus good scores on the Short Physical Performance Battery (OR, 3.59; 95% CI, 2.36-5.47). They also had slower gait speed, less endurance, and lower physical quality of life. VTE survivors were similar to coronary heart disease and stroke survivors on categorical frailty and outcomes on Short Physical Performance Battery assessment. When score on the Short Physical Performance Battery instrument was modeled continuously, VTE survivors performed better than stroke survivors but worse than CHD survivors. Conclusions VTE survivors had triple the odds of frailty and poorer physical function than those without the vascular diseases considered. Their function was somewhat worse than that of CHD survivors, but better than stroke survivors. These findings suggest that VTE patients may benefit from additional efforts to improve postevent physical functioning.Regulation of protein synthesis is an essential step of gene expression. This process is under the control of cis-acting RNA elements and trans-acting factors. Gemin5 is a multifunctional RNA-binding protein organized in distinct domains. The protein bears a non-canonical RNA-binding site, designated RBS1, at the C-terminal end. Among other cellular RNAs, the RBS1 region recognizes a sequence located within the coding region of Gemin5 mRNA, termed H12. Expression of RBS1 stimulates translation of RNA reporters carrying the H12 sequence, counteracting the negative effect of Gemin5 on global protein synthesis. A computational analysis of RBS1 protein and H12 RNA variability across the evolutionary scale predicts coevolving pairs of amino acids and nucleotides. RBS1 footprint and gel-shift assays indicated a positive correlation between the identified coevolving pairs and RNA-protein interaction. The coevolving residues of RBS1 contribute to the recognition of stem-loop SL1, an RNA structural element of H12 that contains the coevolving nucleotides. Indeed, RBS1 proteins carrying substitutions on the coevolving residues P1297 or S1299S1300, drastically reduced SL1-binding. Unlike the wild type RBS1 protein, expression of these mutant proteins in cells failed to enhance translation stimulation of mRNA reporters carrying the H12 sequence. Therefore, the PXSS motif within the RBS1 domain of Gemin5 and the RNA structural motif SL1 of its mRNA appears to play a key role in fine-tuning the expression level of this essential protein.Modern approaches to the Whorfian linguistic relativity question have reframed it from one of whether language shapes our thinking or not, to one that tries to understand the factors that contribute to the extent and nature of any observable influence of language on perception. The current paper demonstrates that such understanding is significantly enhanced by moving the evidentiary basis toward a more biologically grounded empirical arena. We review recent neuroscientific evidence using a variety of methodological techniques that reveal the functional organisation and temporal distribution of the ubiquitous relationship between language and cognitive processing in the human brain.Objective The aim of this study was to determine the effect of web-based education and counselling for patients with systemic lupus erythematosus on self-efficacy, fatigue and assessment of care. Methods The study was conducted as a randomized controlled trial. The study sample consisted of 80 patients divided into two groups the experimental group (n = 40) and a control group (n = 40). Randomization was performed by simple random sampling. At the beginning of the study (month 0), data-collection forms were administered to both groups. Web-based education was carried out for the first three months, and counselling and information updates were given for the next three months for the experimental group. In the intervention process, the control group just received standard care. After six months, data-collection forms were administered to both groups again. Results The mean age of the participants in the experimental and control groups was 35.58 ± 8.40 years and 39.00 ± 12.71 years, respectively. In both groups, 95% of patients were women. Wilcoxon's test was used for within-group comparisons before and after the study. The Mann-Whitney U-test was used to evaluate the difference between the two groups before the intervention and between the two groups after the intervention. We found that there was a significant improvement in fatigue, self-efficacy and assessment of chronic illness care in the experimental group at the end of the study (p less then 0.05). Conclusions The intervention had a positive effect on self-efficacy, fatigue and satisfaction with chronic illness. In accordance with the results, similar studies should be conducted for different patient groups in order to strengthen the results.CRISPR/Cas9 is increasingly being used as a tool to prosecute functional genomic screens. However, it is not yet possible to apply the approach at scale across a full breadth of cell types and endpoints. In order to address this, we developed a novel and robust workflow for array-based lentiviral CRISPR/Cas9 screening. We utilized a β-lactamase reporter gene assay to investigate mediators of TNF-α-mediated NF-κB signaling. The system was adapted for CRISPR/Cas9 through the development of a cell line stably expressing Cas9 and application of a lentiviral gRNA library comprising mixtures of four gRNAs per gene. We screened a 743-gene kinome library whereupon hits were independently ranked by percent inhibition, Z' score, strictly standardized mean difference, and T statistic. A consolidated and optimized ranking was generated using Borda-based methods. Screening data quality was above acceptable limits (Z' ≥ 0.5). Some neuronal disorders, such as amyotrophic lateral sclerosis (ALS), severely impairs the communication capacity of patients. Several invasive and non-invasive brain-computer interfaces (BCIs), most notably using electroencephalography (EEG), have been developed to provide a means of communication to paralyzed patients. However, except for a few reports, all available BCI- literature in the paralyzed (mostly ALS patients) describes patients with intact eye movement control, i.e. patients in a locked-in state (LIS) but not completely locked-in state (CLIS). In this article we will discuss 1) The fundamental neuropsychological learning factors and neurophysiological factors determining BCI performance in clinical applications; 2) We define the difference between LIS and CLIS; 3) Recent development in the BCIs for communication in complete locked-in state patients; 4) Effect of BCI based communication on emotional well-being and quality of life; and 5) An outlook and the methodology needed to provide a means of communication to patients who have none. Thus, we present an overview of the available studies and recent results and try to anticipate future developments which may open new doors for BCI communication with the completely paralyzed. All rights reserved.The popular method of calculating the noncovalent interaction energies at the coupled-cluster single-, double-, and perturbative triple-excitations [CCSD(T)] theory level in the complete basis set (CBS) limit was to add a CCSD(T) correction term to the CBS second-order Møller-Plesset perturbation theory (MP2). The CCSD(T) correction term is the difference between the CCSD(T) and MP2 interaction energies evaluated in a medium basis set. GA-017 However, the CCSD(T) calculations with the medium basis sets are still very expensive for systems with more than 30 atoms. Comparatively, the domain-based local pair natural orbital coupled-cluster method [DLPNO-CCSD(T)] can be applied to large systems with over 1,000 atoms. Considering both the computational accuracy and efficiency, in this work, we propose a new scheme to calculate the CCSD(T)/CBS interaction energies. In this scheme, the MP2/CBS term keeps intact and the CCSD(T) correction term is replaced by a DLPNO-CCSD(T) correction term which is the difference between the DLPNO-CCSD(T) and DLPNO-MP2 interaction energies evaluated in a medium basis set. The interaction energies of the noncovalent systems in the S22, HSG, HBC6, NBC10, and S66 databases were recalculated employing this new scheme. The consistent and tight settings of the truncation parameters for DLPNO-CCSD(T) and DLPNO-MP2 in this noncanonical CCSD(T)/CBS calculations lead to the maximum absolute deviation and root-mean-square deviation from the canonical CCSD(T)/CBS interaction energies of less than or equal to 0.28 kcal/mol and 0.09 kcal/mol, respectively. The high accuracy and low cost of this new computational scheme make it an excellent candidate for the study of large noncovalent systems. © 2020 Wiley Periodicals, Inc.Resulting from a community-identified need for a well-validated indicator of caregiving difficulties for use in practice settings, a brief form of the Atypical Maternal Behavior Instrument for Assessment and Classification System (AMBIANCE) was developed for use as a screening instrument. Prior to its dissemination, this study aimed to assess the feasibility, reliability, and validity of the AMBIANCE-Brief. Adolescent mother-infant dyads (N = 69) participated in the Strange Situation Procedure, as well as play sessions with and without toys. Maternal disrupted caregiving was coded from the play sessions using the AMBIANCE and AMBIANCE-Brief. The AMBIANCE-Brief demonstrated convergent validity with the AMBIANCE in the play session with toys (r = .65, p less then .001) and without toys (r = .61, p less then .001). Concurrent validity of the AMBIANCE-Brief was also demonstrated in relation to infant attachment disorganization in the play session with toys (r = .36, p less then .05) and without toys (r = .32, p less then .01). These findings suggest a shorter protocol for assessing disrupted caregiving may be feasible and valid for use in community settings. Future studies are in progress to train community practitioners in the use of the AMBIANCE-Brief and to evaluate their reliability. © 2020 Michigan Association for Infant Mental Health.OBJECTIVES We performed preoperative ultrasonography (US) to detect the anatomic course of the superficial radial nerve (SRN) and dominant pathologic tendon of the first extensor compartment in de Quervain tenosynovitis. METHODS We prospectively studied 27 patients (29 wrists) with de Quervain tenosynovitis who underwent surgical release of the first extensor compartment. Preoperatively, US was performed to evaluate the presence of the dominant pathologic tendon and the septum in the subcompartment, number of SRNs in the area of the surgical incision, and anatomic running course of the SRN. These variables were also checked intraoperatively. Cohen κ statistics were calculated to investigate agreement between US and surgical field findings. RESULTS There were 7 men and 20 women (mean age, 47.8 years; range, 26-67 years).       <description>&lt;p&gt;With 50% ethanol solution, the DNA conformational transition passes through two steps (i) fast and constant-velocity contraction and (ii) relatively slow contraction from the free end. The first process is attributed to the B to A conformational transition by gradual dehydration. The second process is due to the coil-globule transition as the free end of DNA starts the contraction. This globular structure formation counteracts the shear force from the microfluids and decelerates the contraction velocity. This real-time observation system can be applied to the kinetic analysis of DNA conformational transitions such as kinetics of chromatin assembly and gene expression.Developing a noncontact ratiometric luminescent temperature sensor with high sensitivity, widely available emission range, and reliable performance is a challenge in materials science. Herein, we demonstrated that this goal can be achieved by fabricating a lanthanide-functionalized hydrogen-bonded organic framework film (named the Eu@HOF-TCBP film). The unbonded carboxylic groups that existed in the structure not only enable lanthanide ions to bind with the framework for bringing dual emission but also allow for preparing a hydrogen-bonded organic framework (HOF) film through the facile electrophoretic deposition. The obtained film exhibits ratiometric temperature sensing performance in the range of 297-377 K with a maximum relative sensitivity of 5.787% K-1 and shows repeated use without sensitivity loss. Moreover, the material can be easily recycled and refabricated with consistent performance, demonstrating its unique merits of easy recyclability and regeneration as an HOF material. We believe that the reported strategies for preparing a dual-emitting HOF and fabricating a thin film will open a window for HOF applications.We have developed a protocol to facilitate the nickel-catalyzed decarbonyloxidation of 3-aryl benzofuran-2(3H)-ones to 2-hydroxybenzophenones under mild conditions, which is an efficient approach for the decarbonyloxidation of lactones in organic synthesis. A diverse range of substrates can undergo C(O)-O/C(O)-C bond cleavage to generate the target products in good yields. These 2-hydroxybenzophenones can be converted into a variety of compounds via reactions such as esterification, cyclization, and reduction.Aim To determine factors associated with overweight/obesity in preschool children. Methods This cross-sectional study was carried out in paediatric outpatient departments of the Public Institution Primary Health Care Centre of Canton Sarajevo, Bosnia and Herzegovina. The study included 300 preschool children who were divided into two groups normal weight (n=150) and overweight/obese (n=150). Children were weighed and measured and body mass index was calculated. Data concerning potential childhood overweight/obesity risk factors (sociodemographic characteristics of child and mother, physical activity and sedentary behaviour of the child) were collected using a designed questionnaire filled in by the mothers. Logistic regression analysis was used to estimate the association between potential risk factors and overweight/obesity in preschool children. Results Male gender of the children (p=0.043) and maternal overweight (p=0.000) were positively associated with child overweight/obesity. Male children were 1.6 times more likely to be overweight/obese than female (95% CI 1.01-2.53). Children of overweight mothers were 3.34 times more likely to be overweight/ obese than children with mothers of normal weight (95% CI 1.77- 6.28). Physical activity 180 minutes/day (p=0.020), regular snacking while watching television, using computer/tablet/ mobile phones (p=0.000) were associated with overweight/obesity status of the preschool children. Conclusion Public health programs that aim to reduce overweight/ obesity in preschool children should mainly help mothers understand the serious risk of childhood obesity and the importance of them creating a healthy lifestyle in childhood.Aim To compare maternal, foetal and neonatal characteristics, and perinatal outcome of preterm and term deliveries in twins pregnancies in order to improve perinatal care in Bosnia and Herzegovina. Methods This retrospective cohort study included pregnant women with twin pregnancy who delivered during the period between 1 January 2012 and 31 December 2018 at the Clinic for Gynaecology and Obstetrics, University Clinical Centre Tuzla. Results During the seven-year period 26 734 deliveries were recorded, out of which 362 (1.35 %) were twin pregnancies, 226 (62.4%) preterm and 136 (37.5%) term ones. In the preterm group 38 (16.8%) pregnancies were assisted medical reproduction, and 16 (11.7%) of those were in the term group. The average birth weight was significantly higher for the first twin in both groups (p less then 0.00001). Incipient intrauterine foetal asphyxia was more frequent in the preterm group (p less then 0.05). The most common indication for Caesarean section was abnormalities of foetal presentation and lie, 176 (68.2%) for the overall sample. Conclusion Cornerstone of twin pregnancy antenatal care is to get correct data about amnionicity and chorionicity. Since majority of prenatal data did not have this information we call all obstetricians to declare about amnionicity and chorionicity in twin pregnancies during the first trimester ultrasound examination.Aim To determine the degree of accuracy in determining the sex of the skull based on classical morphometric analysis of the mastoid process. Morphometric analysis excluded a subjective approach expressed in osteoscopic analysis. Methods The study was conducted on a sample of 100 macerated skulls of known gender and age from the second half of the 20th century, including the Bosnian population. Of the 100 skulls, 50 (50%) were male and 50 (50%) were female. Male skulls were on average 60 (47-71) years old and female 57 (43-76) years old. These developments call for simple yet valid stochastic models to describe various multisite phosphorylation processes, especially in large and complex biochemical networks. To reduce model complexity, this work aims at simplifying the multisite phosphorylation mechanism by a stochastic Hill function model. Furthermore, this work optimizes regions of parameter space to match simulation results from the stochastic Hill function with the distributive multisite phosphorylation process. While traditional parameter optimization methods have been focusing on finding the best parameter vector, in most circumstances, modelers would like to find a set of parameter vectors that generate similar system dynamics and results. This paper proposes a general α-β-γ rule to return an acceptable parameter region of the stochastic Hill function based on a quasi-Newton stochastic optimization algorithm. Different objective functions are investigated characterizing different features of the simulation-based empirical data, among which the approximate maximum log-likelihood method is recommended for general applications. Numerical results demonstrate that with an appropriate parameter vector value, the stochastic Hill function model depicts the multisite phosphorylation process well except the initial (transient) period.Many measurements have indicated that thin polymer films in their glass state exhibit a mobile interfacial layer that grows in thickness upon heating, while some measurements indicate the opposite trend. Moreover, simulations and limited measurements on glass-forming liquids at temperatures above the glass transition temperature Tg exhibit a growing interfacial mobility scale ξ upon cooling. To better understand these seemingly contradictory trends, we perform molecular dynamics simulations over a temperature regime for which our simulated polymer film enters a non-equilibrium glassy state and find that the relaxation time τα within the film interior, relative to the polymer-air interfacial layer, exhibits a maximum near the computational Tg. Correspondingly, we also observe that the interfacial mobility length scale exhibits a maximum near Tg, explaining the apparent reversal in the temperature dependence of this scale between the glass and liquid states. We show that the non-monotonic variation of ξ and the relative interfacial mobility to the film interior arise qualitatively from a non-monotonic variation of the gradient of the effective activation free energy of the film; we then obtain a quantitative description of this phenomenon by introducing a phenomenological model that describes the relaxation time layer-by-layer in the film for a temperature range both above and below Tg of the film as a whole. This analysis reveals that the non-monotonic trend in the relative interfacial mobility and ξ both arise primarily from the distinctive temperature dependence of relaxation in the interfacial layer, which apparently remains in local equilibrium over the whole temperature range investigated.The properties of molecules and materials containing light nuclei are affected by their quantum mechanical nature. Accurate modeling of these quantum nuclear effects requires computationally demanding path integral techniques. Considerable success has been achieved in reducing the cost of such simulations by using generalized Langevin dynamics to induce frequency-dependent fluctuations. Path integral generalized Langevin equation methods, however, have this far been limited to the study of static, thermodynamic properties due to the large perturbation to the system's dynamics induced by the aggressive thermostatting. Here, we introduce a post-processing scheme, based on analytical estimates of the dynamical perturbation induced by the generalized Langevin dynamics, which makes it possible to recover meaningful time correlation properties from a thermostatted trajectory. We show that this approach yields spectroscopic observables for model and realistic systems that have an accuracy comparable to much more demanding approximate quantum dynamics techniques based on full path integral simulations.In this work, we investigate the Fe3O4 (001) surface/water interface by combining several theoretical approaches, ranging from a hybrid functional method (HSE06) to density-functional tight-binding (DFTB) to molecular mechanics (MM). First, we assess the accuracy of the DFTB method to correctly reproduce HSE06 results on structural details and energetics and available experimental data for adsorption of isolated water, dimers, and trimers up to a water monolayer. Second, we build two possible configurations of a second and a third overlayer and perform molecular dynamics simulations with DFTB, monitoring the water orientation, the H-bond network, and the ordered water structure formation. To make our models more realistic, we then build a 12 nm-thick water multilayer on top of the Fe3O4 (001) surface slab model, which we investigate through MM-molecular dynamics (MD). This study aimed to assess the serum C-reactive protein (CRP), malondialdehyde (MDA), non-enzymatic antioxidant (vitamin C), and trace elements (Zn, Cu, Mn, and Fe) in RA patients, and thereby correlate these parameters with the association of RA. This study included 20 Bangladeshi RA patients and 20 normal healthy volunteers as control subjects. CRP level was determined using a laboratory-based latex agglutination-enhanced immunoassay. The lipid peroxidation level was determined by measurement of the serum level of MDA. Non-enzymatic antioxidant vitamin C was assessed by UV spectrophotometric method. Trace elements were determined by atomic absorption spectroscopy (AAS). Our study observed significantly higher concentrations of CRP (p  less then  0.001) and MDA (p  less then  0.001), and significantly lower concentrations of vitamin C (p  less then  0.001) in the RA patient. The mean values of Zn, Cu, Mn, and Fe were 6.62 ± 0.34, 1.42 ± 0.17, 7.51 ± 0.23, and 29.25 ± 0.41 ppm for the RA patients respectively and 13.57 ± 9.13, 1.15 ± 0.17, 1.59 ± 0.18, and 62.47 ± 5.25 ppm for the control subjects, consequently. There was a significant difference (p  less then  0.05) in the trace element levels between the RA patients and control subjects. The present study aimed to investigate the total mercury (Hg) concentrations in hair samples of 40 wild cats belonging to eight different species collected from various provinces of Iran and to characterize their risk of Hg exposure. Total Hg levels in Iranian wild cats ranged from 62 to 3670 ng/g dw hair, with a median value of 488 ng/g dw hair. The lowest median Hg concentration was found in west Iran (251 ng/g dw hair), while the highest median level was measured in the north-east of the country (736 ng/g dw hair), likely related to its past use in agriculture and potential contamination of industrial effluents. The overall contamination of Iranian wild cats with mercury can be considered generally low, but 20% of the samples reached levels above 1100 ng/g dw hair, set as indicative of an environmental Hg concern according to the U.S. Fish and Wildlife Service. Our data are comparable with other investigations on wildlife worldwide, confirming the suitability of museum collection specimens to assess the environmental levels of Hg.
