Palliative care is a holistic approach to care that includes symptom management and goal setting to improve patients' quality of life. Palliative care is designed to be used with patients who have chronic illness that impacts quality of life and should be incorporated early in the disease. All providers have a responsibility to be knowledgeable in palliative care approaches and to know when to refer a patient for specialty palliative care services. This article will describe palliative care, research study findings, and current professional guideline recommendations for patients. The article also describes challenges and barriers to the use of palliative care in heart transplant patients and strategies to address these challenges and barriers.CONTEXT Early tailored transplant education could help patients make informed transplant choices. OBJECTIVE We interviewed 40 patients with chronic kidney disease (CKD) stages 3 to 5, 13 support persons, and 10 providers at Kaiser Permanente Southern California to understand (1) barriers to transplant education and (2) transplant educational preferences and recommendations based on CKD stage and primary language spoken. DESIGN A grounded theory analysis identified central themes related to transplant education barriers, preferences, and recommendations. RESULTS Barriers included confusion about diagnosis and when transplant may be necessary, concerns about transplant risks, families' lack of transplant knowledge, financial burdens, transportation and scheduling, and the emotional overload of chronic illness. Hispanic and Spanish-speaking participants reported difficulty in understanding transplant education and medical mistrust. Recommendations included providing general education, earlier introduction to transplant, wait-listing information, transplant education for support persons, living donation education for patients and potential donors, opportunities to meet living donors and kidney recipients, information on the benefits of transplant, recovery, and available financial resources, flexible class scheduling, online and print resources, and more provider follow-up. Spanish-speaking and Hispanic participants recommended using bilingual educators, print, video, and online resources in Spanish, and culturally responsive education. Patients with CKD stages 3 to 4 wanted information on slowing disease progression and avoiding transplant. CONCLUSION Increasing access to culturally responsive transplant education in multiple languages, pairing appropriate content to the disease stage, and increasing system-wide follow-up as the disease progresses might help patients make more informed choices about transplant.Aim of the study With the development of emergency medicine and intensive care technology, the number of people who survive with disorders of consciousness (DOC) has dramatically increased. The diagnosis and treatment of such patients have attracted much attention from the medical community. From the latest evidence-based guidelines, non-invasive brain intervention (NIBI) techniques may be valuable and promising in the diagnosis and conscious rehabilitation of DOC patients.Methods This work reviews the studies on NIBI techniques for the assessment and intervention of DOC patients.Results A large number of studies have explored the application of NIBI techniques in DOC patients. The NIBI techniques include transcranial magnetic stimulation, transcranial electric stimulation, music stimulation, near-infrared laser stimulation, focused shock wave therapy, low-intensity focused ultrasound pulsation and transcutaneous auricular vagus nerve stimulation.Conclusions NIBI techniques present numerous advantages such as being painless, safe and inexpensive; having adjustable parameters and targets; and having broad development prospects in treating DOC patients.INTRODUCTION Depression and anxiety are common affective experiences during the first year following a solid organ transplant. This study examined the degree to which an evidenced-based clinical intervention implemented by social workers-Symptom Targeted Intervention-can alter self-reported depression and anxiety in heart, kidney, liver, and lung transplant recipients. RESEARCH QUESTIONS This investigation explored 2 questions (1) Can symptom-targeted interventions significantly reduce posttransplant recipients' self-reported depression and anxiety at the conclusion of treatment and at 1-month follow-up? and (2) Does the response differ by gender? DESIGN A 1-group pretest-posttest design with a 1-month follow-up was used to test for changes in anxiety and depression after transplantation. Here, we report health-related quality of life (HRQoL) results from this trial. Patients received PVd or Vd in 21-day cycles until disease progression or discontinuation. HRQoL was assessed using the EORTC QLQ-C30, QLQ-MY20, and EQ-5D-3L instruments on day 1 of each treatment cycle. Mean score changes for global QoL, physical functioning, fatigue, side effects of treatment domains, and EQ-5D-3L index were generally stable over time across treatment arms. The applicability of these scores in patients with evidence of myocardial ischemia but no obstructive coronary artery disease is unclear. Methods and Results Among 935 women with signs and symptoms of ischemia enrolled in WISE (Women's Ischemia Syndrome Evaluation), 567 had no obstructive coronary artery disease on angiography. Of these, 433 had had available risk data for 6 commonly used scores Framingham Risk Score, Reynolds Risk Score, Adult Treatment Panel III, Atherosclerotic Cardiovascular Disease, Systematic Coronary Risk Evaluation, Cardiovascular Risk Score 2. Score-specific CVD rates were assessed. For each score, we evaluated predicted versus observed event rates at 10-year follow-up using c statistic. Recalibration was done for 3 of the 6 scores. The 433 women had a mean age of 56.9±9.4 years, 82.5% were white, 52.7% had hypertension, 43.6% had dyslipidemia, and 16.9% had diabetes mellitus. The observed 10-year score-specific CVD rates varied between 5.54% (Systematic Coronary Risk Evaluation) to 28.87% (Framingham Risk Score), whereas predicted event rates varied from 1.86% (Systematic Coronary Risk Evaluation) to 6.99% (Cardiovascular Risk Score 2). The majority of scores showed moderate discrimination (c statistic 0.53 for Atherosclerotic Cardiovascular Disease and Systematic Coronary Risk Evaluation; 0.78 for Framingham Risk Score) and underestimated risk (statistical discordance -58% for Adult Treatment Panel III; -84% for Atherosclerotic Cardiovascular Disease). Recalibrated Reynolds Risk Score, Atherosclerotic Cardiovascular Disease, and Framingham Risk Score had improved performance, but significant underestimation remained. Conclusions Commonly used CVD risk scores fail to accurately predict CVD rates in women with ischemia and no obstructive coronary artery disease.       <description>&lt;p&gt;BACKGROUND Hypertension, a common chronic disease, is associated with cognitive impairment. Cognitive impairment, especially Alzheimer's disease (AD), seriously affects older adults' quality of life and aggravates the burden of disease on society and families. Elevated Alzheimer-associated neuronal thread protein (AD7c-NTP) has been observed in the urine of patients with AD and mild cognitive impairment; however, it is not clear whether this protein can be used as a biomarker for cognitive impairment in older hypertensive patients. OBJECTIVE To explore the value of urinary AD7c-NTP, and the association of urinary AD7c-NTP with cognitive function in older hypertensive patients. METHODS This was a cross-sectional study. In total, 134 hypertensive patients aged ≥60 years were divided into two groups Lower Cognitive Function group (LCF group, n = 89) and a Normal Control group (NC group, n = 45) based on the Montreal Cognitive Assessment (MoCA). Urinary AD7c-NTP, blood glucose, serum insulin, superoxide dismutase (SOD) and malondialdehyde (MDA) levels were measured. RESULTS Urinary AD7c-NTP level was significantly higher in the LCF group than in the NC group [0.48 (0.21-1.00) versus 0.25 (0.04-0.44) ng/ml, p  less then  0.001]. SOD level [(43.07±23.74) versus (53.12±25.80) U/ml, p = 0.026] and higher homeostasis model assessment of insulin resistance (HOMA-IR) [7.17 (3.74-13.94) versus 6.01 (3.78-7.43), p = 0.033] was lower in the older hypertensive patients than in the NC group. Urinary AD7c-NTP level was associated with MoCA score and HOMA-IR but not with SOD, MDA, blood glucose, and insulin. CONCLUSION The level of urinary AD7c-NTP is elevated in older hypertensive patients with lower cognitive function, and insulin resistance may be involved in the process.As the elderly population grows, chronic metabolic dysfunction including obesity and diabetes are becoming increasingly common comorbidities. Hypothalamic inflammation through CNS resident microglia serves as a common pathway between developing obesity and developing systemic aging pathologies. Despite understanding aging as a life-long process involving interactions between individuals and their environment, limited studies address the dynamics of environment interactions with aging or aging therapeutics. We previously demonstrated environmental enrichment (EE) is an effective model for studying improved metabolic health and overall healthspan in mice, which acts through a brain-fat axis. Here we investigated the CSF1R inhibitor PLX5622 (PLX), which depletes microglia, and its effects on metabolic decline in aging in interaction with EE. PLX in combination with EE substantially improved metabolic outcomes in middle-aged female mice over PLX or EE alone. Chronic PLX treatment depleted 75% of microglia from the hypothalamus and reduced markers of inflammation without affecting brain-derived neurotrophic factor levels induced by EE. Adipose tissue remodeling and adipose tissue macrophage modulation were observed in response to CSF1R inhibition, which may contribute to the combined benefits seen in EE with PLX. Our study suggests benefits exist from combined drug and lifestyle interventions in aged animals.The AKT/mTOR pathway is critical for bladder cancer (BC) pathogenesis and is hyper-activated during BC progression. In the present study, we identified a novel positive feedback loop involving oncogenic factors histone methyltransferase SMYD3, insulin-like growth factor-1 receptor (IGF-1R), AKT, and E2F-1. Selleck AT13387 SMYD3 expression was significantly up-regulated in BC tumors and positively associated with histological grade, lymph node metastasis, and shorter patient survival. Depletion of SMYD3 inhibited BC cell proliferation, colony formation, migration, invasion, and xenograft tumor growth. Mechanistically, SMYD3 inhibition led to the diminished AKT/mTOR signaling activity, thereby triggering deleterious effects on BC cells. Furthermore, SMYD3 directly activates the expression of IGF-1R, a critical activator of AKT in BC, by inducing hyper-methylation of histone H3-K4 and subsequent chromatin remodeling in the IGF-1R promoter region. On the other hand, E2F-1, a downstream factor of the AKT pathway, binds to the E2F-1 binding motifs at the SMYD3 promoter and consequently induces SMYD3 transcription and expression. Thus, SMYD3/IGF-1R/AKT/E2F-1 forms a positive feedback loop leading to the hyper-activated AKT signaling. Our findings provide not only profound insights into SMYD3-mediated oncogenic activity but also present a unique avenue for treating BC by directly disrupting this signaling circuit.Objectives To test the hypothesis that scores on a Grit scale are positively associated with personality measures that are conducive to relationship building (Empathy, Self-Esteem, Activity, and Sociability), but inversely associated with personality measures that are detrimental to interpersonal relationships (Neuroticism-Anxiety, Aggression-Hostility, Impulsive Sensation Seeking, and Loneliness). Methods Convenient sampling was used that included 241 medical students at Sidney Kimmel Medical College at Thomas Jefferson University who participated in this ex post facto research. Validated instruments were used to measure Grit, Empathy, Self-Esteem, Activity, Sociability, Neuroticism-Anxiety, Aggression-Hostility, Impulsive Sensation Seeking, and Loneliness. Bivariate correlations and multivariate regression were used to examine relationships between scores on the Grit scale and personality measures. Results Results of bivariate correlational analyses showed that scores on the Grit scale were positively and significantly (p less then 0.01) correlated with measures of Self-Esteem (r=0.35), Empathy (r=0.26), and Activity (r=0.17); but negatively and significantly (p less then 0.01) correlated with measures of Loneliness (r=-0.28), Aggression-Hostility (r=-0.23), Neuroticism-Anxiety (r=-0.22), and Impulsive Sensation Seeking (r=-0.18). Regression analysis indicated that in a multivariate model, higher scores on Self-Esteem and Empathy and lower scores on Aggression-Hostility were uniquely and significantly associated with Grit scores (R=0.43, p less then 0.01). Conclusions Research hypothesis was partially confirmed, suggesting that medical students with higher Grit scores were likely to have higher empathic orientation in patient care and greater Self-Esteem. Conversely, those with higher degrees of Grit displayed lower levels of Aggression-Hostility and Impulsive Sensation Seeking. In order to understand the role of CXC chemokine family in carcinogenesis, this study explored a group of early gastric cancer (GC) patients, and assessed the level of CXC chemokine ligand (CXCL) in blood samples of patients representing systemic circulation and tumor microenvironment, detected the expression of CXC chemokine receptor (CXCR) in tumor tissues, and measured tumor infiltrating immune cell subsets. 69 patients with GC were included in a single center prospective study and were followed up for 6 years. The level of CXCL1-14 was determined by ELISA and the concentration gradient of chemokine was calculated. Western blot was used to detect the expression of CXCR1, CXCR2, CXCR3, and CXCR4 in tumor tissue. We present a case of a 65-year-old Chinese man with a three-month history of cough and expectoration. The diagnosis was a combination of poorly differentiated synchronous central type primary lung sarcomatous carcinoma and lung squamous cell carcinoma with approximately 50% proportion of each, and the TNM classification was pT2aN1M0, at least AJCC Stage IIA. IJCEP Copyright © 2020.Radiofrequency ablation (RFA) is a potentially curative therapy for nontransplantable hepatocellular carcinoma (HCC). However, as tumor size increases, incomplete RFA can increase rates of local recurrence and tumor progression. As such, there remains a need to identify potential biologic mechanisms mediating HCC response to thermal ablation. Our results revealed that miR-103 was markedly upregulated in recurrent HCC tissues treated with RFA as first-line treatment and in HCC lines after heat stress in vitro, simulating the marginal zone of RFA treatment. Gain-of-function and loss-of-function studies showed that miR-103 ectopic overexpression promoted, but miR-103 silencing reduced, heat-exposed HCC proliferation, and migration in vitro. Western blotting displayed that proteins related with proliferation and migration were significantly changed in different groups. Furthermore, PTEN may be a potential target of miR-103 and miR-103 could activate the PI3K/Akt pathway by suppressing PTEN expression. Taken together, these studies provide experimental evidence supporting a role for miR-103 in HCC response to heat stress. IJCEP Copyright © 2020.Background and objective To further determine the association between mucin 1 (MUC1) rs4072037 polymorphism and gastric cancer risk on the basis of previously published studies. Methodology PubMed and Embase were used to search all the available publications. The relative risk of the correlation was shown as odds ratio (OR) with 95% confidence interval (95% CI) under all the genetic comparisons. Subgroup analyses based on ethnicity, study design and HWE were also executed to detect effects of specific factors on the risk of gastric cancer. Results MUC1 rs4072037 polymorphism was observed to reduce the risk of gastric cancer under the five genetic comparisons [(GG versus AA OR (95% CI)=0.72 (0.61, 0.84); GG + GA versus AA OR (95% CI)=0.82 (0.76, 0.88); GG versus AA + GA OR (95% CI)=0.83 (0.71, 0.96); G versus A OR (95% CI)=0.78 (0.72, 0.84); GA versus AA OR (95% CI)=0.80 (0.74, 0.87)]. This decreased risk of gastric cancer was also detected in subgroup analyses based on ancestry (Asian and Caucasian), study design (population-based and hospital-based) and HWE (P HWE&amp;gt;0.05). Conclusions MUC1 rs4072037 polymorphism may have an important role in gastric cancer, and this protective effect may vary among different ethnic populations and control subjects. P53 can regulate many cells signaling pathways and play an important role in cell cycle, cell apoptosis, and cell senescence. However, there are few reports on the expression of EZH2 and p53 in ovarian cancer and their correlation with the ovarian cancer. The purpose is to elucidate the expression of EZH2 and p53 in ovarian cancer and to study the relationship of EZH2 and p53 with the clinical parameters of ovarian cancer. In this study, both mRNA and protein level of EZH2 in ovarian cancer group was significantly higher than that in borderline, benign, and normal group; while the mRNA and protein level of p53 was significantly lower than that in borderline, benign, and normal group.
