Building an Incident Response Agent That Remembers with Hindsight

Introduction

In software development, production incidents are unavoidable. A server may suddenly become unavailable, a database connection may fail, an API may stop responding, or an application may experience unexpected performance issues.

When these problems occur, DevOps engineers must identify the cause, investigate logs, find a solution, and restore the service as quickly as possible.

However, there is one important problem: teams often solve similar incidents repeatedly because the knowledge gained from previous incidents is not always available when a new problem occurs.

What if an AI agent could remember previous incidents, understand how they were resolved, and use that experience to help engineers handle similar problems faster?

This idea inspired our Incident Response Agent, an AI-powered Engineering and DevOps assistant that uses Hindsight as its memory layer.

The Problem: Incident Response Without Memory

Traditional incident response often depends on logs, monitoring dashboards, documentation, and the experience of individual engineers.

Consider this example:

A production application suddenly reports database connection timeouts.

An engineer investigates the issue, discovers that the database connection pool is exhausted, increases the connection limit, and restores the application.

A few weeks later, the same issue occurs again.

The team may need to investigate the same symptoms and search through old conversations, documents, and incident reports to find the previous solution.

This creates several challenges:

Repeated troubleshooting takes valuable time.

Previous solutions can be difficult to find.

Important knowledge may remain with individual engineers.

Runbooks and documentation may become outdated.

New team members may not know how earlier incidents were resolved.

The problem is not necessarily a lack of information. It is the difficulty of retrieving and applying relevant experience at the right time.

Our Solution: An Incident Response Agent

We designed an AI-powered Incident Response Agent that can use historical incident information to support troubleshooting.

Instead of treating every incident as a completely new problem, the agent can use relevant information from previous incidents.

The main idea is to give the agent a memory system that stores useful incident knowledge and retrieves it when similar problems occur.

The system is designed around four important activities:

Incident collection: Receive incident details such as error messages, symptoms, affected services, and logs.

Memory storage: Store useful incident information, including root causes, resolutions, and lessons learned.

Memory retrieval: Search for relevant previous incidents when a new issue appears.

AI-assisted recommendations: Use the retrieved information to help engineers investigate and choose possible next steps.

Why Hindsight?

An ordinary AI conversation does not automatically provide a reliable long-term record of every previous incident.

For an incident response system, remembering useful information across different troubleshooting sessions is important.

This is where Hindsight comes into the picture.

Hindsight provides a memory layer that an AI agent can use to retain and recall information.

In our project, the intended memory workflow is:

Retain: Store important information from resolved incidents and post-mortems.

Recall: Retrieve relevant information when a new incident needs investigation.

For example, after resolving a database connection issue, the agent can retain the incident symptoms, root cause, resolution steps, and verification results.

When another database connection issue occurs, the agent can recall the earlier incident and use it as supporting context.

This allows the system to make use of previous troubleshooting knowledge rather than starting from zero every time.

System Architecture

The proposed architecture contains the following components:

Incident Input Layer

The engineer provides incident information, such as an error message, service name, severity, and relevant logs.

AI Agent

The agent interprets the incident, identifies useful information, and determines whether historical context is needed.

Hindsight Memory Layer

Hindsight stores and retrieves historical incident knowledge through retain and recall operations.

Recommendation Layer

The agent combines the current incident details with relevant historical information to suggest investigation steps and possible resolutions.

Engineer Review

The engineer reviews the recommendations, performs approved troubleshooting actions, and confirms whether the issue has been resolved.

The engineer remains responsible for important production decisions.

Example: Handling a Database Connection Incident

Let's consider a practical example.

First incident

An application reports:

"Database connection timeout"

The engineer investigates and identifies the following:

Service: Payment API

Problem: Database connection pool exhaustion

Root cause: Too many concurrent database requests

Resolution: Adjust connection pool configuration and review request handling

Verification: Confirm that the application can establish database connections successfully

After the incident is resolved, the useful information can be retained in Hindsight.

A similar incident occurs later

A new incident reports:

"Database connection timeout in Payment API"

The agent can use Hindsight to recall relevant historical information.

It may find that a previous incident involved connection pool exhaustion and retrieve the associated root cause and resolution steps.

The agent can then present the earlier incident as relevant context and suggest checks for the engineer to perform.

This does not mean that every incident has the same cause. The previous incident is a useful reference, not proof of the current root cause.

How the Memory Workflow Helps

Without historical memory, an engineer may need to search through old incident reports and documentation manually.

With a memory-enabled agent, the workflow becomes:

Receive the current incident.

Understand its symptoms.

Recall similar historical incidents.

Compare previous root causes and resolutions with the current evidence.

Present relevant troubleshooting suggestions.

Allow the engineer to verify the solution.

Retain useful information after the incident is resolved.

The most important benefit is continuity of knowledge between incidents.

Important Design Considerations

An incident response agent must be designed carefully because production systems are sensitive.

Accuracy: The agent should distinguish between confirmed facts and possible explanations.

Human supervision: Critical actions such as restarting services, changing configurations, or modifying databases should require appropriate authorization and human approval.

Security: Logs and incident records may contain confidential information, credentials, or customer data. Sensitive information should not be stored unnecessarily.

Reliable memory: Incorrect or outdated incident information can lead to poor recommendations. Stored knowledge should be reviewed and updated.

Context relevance: Similar error messages do not always indicate identical root causes. Recommendations must be based on the current evidence.

Lessons and Takeaways

Building this project highlights several important ideas:

AI agents become more useful when they can access relevant information from previous tasks.

Long-term memory can help preserve operational knowledge across troubleshooting sessions.

Historical incidents can provide useful context, but they should not replace current investigation.

Human approval is important when an AI system interacts with production infrastructure.

Good incident documentation is essential for creating useful and reliable memory.

Conclusion

Incident response is not only about identifying and fixing technical problems. It is also about preserving the knowledge gained from solving them.

Our Incident Response Agent explores how Hindsight can provide a memory layer for an AI-powered DevOps assistant.

By retaining resolved incident information and recalling relevant experiences during future investigations, the system aims to make troubleshooting more consistent and knowledge more accessible.

The larger idea is simple: an AI agent should not only help solve today's incident; it should also learn from the knowledge gained so that tomorrow's incident can be approached more intelligently.
