so I don't really know anything about coding but I was wondering if there was a way to create a form type file format but offline -not hosted like several online forms- where you collect answers to a question as text or images.
and I was thinking it should be graphically customizable and easy to make (drag and drop or graphic/vector design) just like a pdf but with space for input like forms, so it can be downloaded.

or if that type of thing already exists.
