Socio-economic (SE) development has resulted in increased prevalence of obesity across all social groups in developing countries that is contrary to the effects of rising SE status on prevalence of obesity in the developed world. Obesity is not only associated with metabolic syndrome, cardiovascular disease, diabetes but is also a risk factor for cancer and is responsible for increased cancer mortality. Published articles have reported higher rates of treatment failure and adverse events (AEs) of anti-cancer therapy in obese patients with carcinoma cervix in comparison to their normal body mass index (BMI) counterparts. Hence, there is a need to elucidate factors that may increase the risk of AEs. Aim of this paper is to discuss the delivery of radiotherapy, concurrent chemotherapy and their effect on AEs in obese patients with carcinoma cervix. © 2019 Greater Poland Cancer Centre. The aim is to deliver adequate doses of radiation to kill cancer cells and the most challenging part during this procedure is to protect normal cells from radiation. Researchers have pursued cerium oxide nanoparticles as a therapeutic agent, due to its diverse characteristics, which include antioxidant properties, making it a potential radioprotector. Materials and methods One hundred rats were divided into five groups of A) control group, intraperitoneal (IP) saline injection was done twice a week; B) bi-weekly IP injection of 14.5 nM (0.00001 mg/kg) CNP for two weeks; C) a single whole thorax radiation dose of 18 Gy; D) a single whole thorax radiation dose of 18 Gy + bi-weekly injection of 14.5 nM CNP for two weeks after radiation; E) bi-weekly IP injection of 14.5 nM CNP for two weeks prior to radiation + a single whole thorax radiation dose of 18 Gy. Thirty days after irradiation, 7 rats from each group were anesthetized and their lungs extracted for histopathological examination. Results Statistical analyses revealed that CNP significantly decreased the incidence of tissue collapse and neutrophile aggregation in rats receiving CNP before radiation in comparison with the radiation group. Conclusion The results suggested the possibility of using CNP as a future radioprotector due to its ability to protect normal cells against radiation-induced damage. © 2020 Greater Poland Cancer Centre.       <description>&lt;p&gt;Previous voxel-based morphometry (VBM) and cortical thickness (CT) studies on Parkinson's disease (PD) have mainly reported the gray matter size reduction, whereas the shape of cortical surface can also change in PD patients. For the first time, we analyzed sulcal depth (SD) patterns in PD patients by using whole brain region of interest (ROI)-based approach. In a cross-sectional study, high-resolution brain structural MRI images were collected from 60 PD patients without dementia and 56 age-and sex-matched healthy controls (HC). SD and CT were estimated using the Computational Anatomy Toolbox (CAT12) and statistically compared between groups on whole brain ROI-based level using statistical parametric mapping 12 (SPM12). Additionally, correlations between regional brain changes and clinical variables were also examined. Compared to HC, PD patients showed lower SD in widespread regions, including temporal (the bilateral transverse temporal, the left inferior temporal, the right middle temporal and the right superior temporal), insular (the left insula), frontal (the left pars triangularis, the left pars opercularis and the left precentral), parietal (the bilateral superior parietal) and occipital (the right cuneus) regions. For CT, only the left pars opercularis showed lower CT in PD patients compared to HC. No regions showed higher SD or CT in PD patients compared to HC. In PD patients, a significant positive correlation was found between SD of the left pars opercularis and MMSE scores, such that lower MMSE scores were related to lower SD of the left pars opercularis. Our results of widespread lower SD, but relatively localized lower CT, indicate that SD seems to be more sensitive to brain changes than CT and may be mainly affected by white matter damage. Hence, SD may be a more promising indicator to investigate the surface shape changes in PD patients. The significant positive correlation between SD of the left pars opercularis and MMSE scores suggests that SD may be prognostic of future cognitive decline. &lt;br&gt;
 The objective of the study was to evaluate the prevalence of anxiety and depression among medical trainees during the initial wave of the COVID-19 pandemic. &lt;/p&gt;

&lt;p&gt;This was a cross-sectional study of the prevalence of depression and anxiety among medical students, residents, and fellows at a medical university hospital in New York using self-reported PHQ-9 and GAD-7 screening tools administered via an anonymized survey. The study was conducted in April 2020. &lt;/p&gt;

&lt;p&gt;The authors received 438 responses (33.4% response rate). Nearly half (44.5%) were medical students and female (56.6%). The prevalence of positive screen for depression (45.3%) and anxiety (48.1%) was high. Many reported moderate to severe depression (17.2%) and anxiety (20.3%). Over half (57.3%) experienced significant mood changes and inability to concentrate, and 14.6% had reconsidered their choice of profession since the start of the pandemic. Those who had reconsidered their profession had higher PHQ-9 [8.1 (6.4) vs 4.4 (4.3), p &amp;lt; 0.0001] and GAD-7 scores [8.3 (6.1) vs 4.7 (4.6), p &amp;lt; 0.0001], indicating adverse mental health partly contributed to their reconsideration of choice of profession. Women were more likely to screen positive for anxiety (OR 1.68) and medical students more likely to screen positive for anxiety (OR 2.55) and depression (OR 2.74). &lt;/p&gt;

 During anesthesiologists' careers, a leave of absence (LOA) is common. After prolonged leave, updating may be beneficial in reducing concerns about knowledge and skill decrements. Although formal return-to-work (RTW) courses and checklists assist UK practitioners, and Australia mandates a one-month RTW program for each year away from practice, no Canadian RTW programs exist. This project aimed to determine the needs of anesthesiologists for an RTW program. &lt;/p&gt;

&lt;p&gt;This quality improvement activity developed a needs analysis survey that was sent to all practicing anesthesiologists in Alberta. Respondents provided their opinions about the requirements necessary for an RTW program. &lt;/p&gt;

&lt;p&gt;Seventy-three of 350 eligible participants (21%) responded; one-third of respondents were female. Thirty-four respondents (47%) had taken at least one LOA, with a median [interquartile range] duration of 6 [3-12] months. Overall, respondents thought the duration of an LOA requiring formal RTW updating should be 12 [6-15] months, with a n anesthesiologists and provided initial guidance in the design of a user-centred RTW program. &lt;br&gt;
 Flexible bronchoscope-guided tracheal intubation through supraglottic airway devices (SGAs) is a well-established element of difficult intubation algorithms. Success can be limited by dimensional incompatibilities between tracheal tubes (TTs) and SGAs. &lt;/p&gt;

&lt;p&gt;In this in vitro study, we tested the feasibility of TT passage through SGAs, removal of SGAs over TTs, and the ability to guide the flexible bronchoscope with 13 TT brands (internal diameter, 6.5-8.0 mm) and ten different SGAs (#4 and #5) in an intubation mannequin. &lt;/p&gt;

&lt;p&gt;We tested 1,040 combinations of SGAs and TTs. Tracheal tube passage failed in 155 (30%) combinations of the five tested first-generation SGAs (117 [46%] with SGA #4, 38 [15%] with SGA #5) and in three (0.6%) combinations of the five tested second-generation SGAs (two [0.8%] with SGA #4 and one [0.4%] with SGA #5). The reason for failed passage of a TT through a first-generation SGA consistently was a too-narrow SGA connector. Removal of the SGA over the TT in the 882 remaining combinations was impossible for all sizes of reinforced TTs, except the Parker Reinforced TT, and was possible for all non-reinforced TTs. Only one combination with SGA #4 and 84 combinations with SGA #5 were not ideal to adequately guide the flexible bronchoscope. &lt;/p&gt;

      <description>&lt;p&gt;The management of chronic liver diseases (CLDs) and cirrhosis is associated with substantial healthcare costs. We aimed to estimate trends in national healthcare spending for patients with CLDs or cirrhosis between 1996 and 2016 in the United States. &lt;/p&gt;

&lt;p&gt;National-level healthcare expenditure data developed by the Institute for Health Metrics and Evaluations for the Disease Expenditure Project and prevalence of CLDs and cirrhosis derived from the Global Burden of Diseases Study were used to estimate temporal trends in inflation-adjusted US healthcare spending, stratified by setting of care (ambulatory, inpatient, emergency department, and nursing care). Joinpoint regression was used to evaluate temporal trends, expressed as annual percent change (APC) with 95% confidence intervals (CIs). Drivers of change in spending for ambulatory and inpatient services were also evaluated. &lt;/p&gt;

&lt;p&gt;Total expenditures in 2016 were $32.5 billion (95% CI, $27.0-$40.4 billion). Over 65% of spending was for inpatient or emergency department care. From 1996 to 2016, there was a 4.3%/year (95% CI, 2.8%-5.8%) increase in overall healthcare spending for patients with CLDs or cirrhosis, driven by a 17.8%/year (95% CI, 14.5%-21.6%) increase in price and intensity of hospital-based services. Total healthcare spending per patient with CLDs or cirrhosis began decreasing after 2008 (APC -1.7% [95% CI, -2.1% to -1.2%]), primarily because of reductions in ambulatory care spending (APC -9.1% [95% CI, -10.7% to -7.5%] after 2011). &lt;/p&gt;

He was prescribed first-line systemic treatment with sunitinib (Sutent) 50 mg per day (each cycle 4 weeks on, 2 weeks off). Upon day 22 of his second sunitinib cycle, he came to the oncology clinic complaining of difficulty walking due to bilateral sole pain. He described initial tingling sensations, which then became burning and painful, with symmetrical erythema and edema of the soles, without blisters. These turned into painful plaques with yellowish discoloration and hyperkeratosis on pressure-bearing areas. He denied fever or other symptoms. The pain limited his instrumental activities of daily living, but not his self-care activities of daily living. Total body skin examination disclosed hyperkeratotic plaques on the undersurface of the great toes and heels of both feet, predominantly at sites of pressure; no blisters, crusts, ulcers, or fissures were found. A search of the digital archive (going back to 01/01/1995) of Sheba Medical Center identified hospitalized patients below the age of 40. &lt;/p&gt;

&lt;p&gt;The database was composed of the records of 750 patients (612 males, 138 females) who were hospitalized. The recurrence risk for SP after nonoperative treatment was significantly higher. Women were found to have an increased risk of SSP when having SP (OR 2.78). Asthma was the most prevalent disease causing SSP in young people. &lt;/p&gt;

&lt;p&gt;In this large cohort, we found that operative procedure has clear protective effect from recurrence in SP, so surgery should be positively considered when treating SP in hospitalized patients.       <description>&lt;p&gt;Cells undergo proliferation and apoptosis, migration and differentiation via a number of cell surface receptors, most of which are heavily glycosylated. This review discusses receptor glycosylation and the known roles of glycans on the functions of receptors expressed in diverse cell types. We included growth factor receptors that have an intracellular tyrosine kinase domain, growth factor receptors that have a serine/threonine kinase domain, and cell-death-inducing receptors. N- and O-glycans have a wide range of functions including roles in receptor conformation, ligand binding, oligomerization, and activation of signaling cascades. A better understanding of these functions will enable control of cell survival and cell death in diseases such as cancer and in immune responses.Endometrosis is a reproductive pathology that is responsible for mare infertility. Our recent studies have focused on the involvement of neutrophil extracellular traps enzymes, such as elastase (ELA), in the development of equine endometrosis. Noscapine (NOSC) is an alkaloid derived from poppy opium with anticough, antistroke, anticancer, and antifibrotic properties. The present work investigates the putative inhibitory in vitro effect of NOSC on collagen type I alpha 2 chain (COL1A2) mRNA and COL1 protein relative abundance induced by ELA in endometrial explants of mares in the follicular or mid-luteal phases at 24 or 48 h of treatment. The COL1A2 mRNA was evaluated by qPCR and COL1 protein relative abundance by Western blot. In equine endometrial explants, ELA increased COL 1 expression, while NOSC inhibited it at both estrous cycle phases and treatment times. These findings contribute to the future development of new endometrosis treatment approaches. Noscapine could be a drug capable of preventing collagen synthesis in mare's endometrium and facilitate the therapeutic approach.Type 2 diabetes is a chronic disease widespread throughout the world, with significant human, social, and economic costs. The present review aims to provide an overview of the current knowledge concerning the molecular crosstalk that takes place between oxidative stress and microRNAs in the physiopathology of type 2 diabetes, with a special emphasis on its potential as a therapeutic target.An ongoing epidemic of chronic kidney disease of uncertain etiology (CKDu) afflicts large parts of Central America and is hypothesized to be linked to heat stress at work. Mortality rates from CKDu appear to have increased dramatically since the 1970s. To explore this relationship, we assessed trends in maximum and minimum temperatures during harvest months between 1973 and 2014 as well as in the number of days during the harvest season for which the maximum temperature surpassed 35 °C. Data were collected at a weather station at a Nicaraguan sugar company where large numbers of workers have been affected by CKDu. Monthly averages of the daily maximum temperatures between 1996 and 2014 were also compared to concurrent weather data from eight Automated Surface Observing System Network weather stations across Nicaragua. Our objectives were to assess changes in temperature across harvest seasons, estimate the number of days that workers were at risk of heat-related illness and compare daily maximum temperatures across various sites in Nicaragua. The monthly average daily maximum temperature during the harvest season increased by 0.7 °C per decade between 1973 and 1990. The number of days per harvest season with a maximum temperature over 35 °C increased by approximately five days per year between 1974 and 1990, from 32 days to 114 days. Between 1991 and 2013, the number of harvest days with a maximum temperature over 35 °C decreased by two days per year, and the monthly average daily maximum temperature decreased by 0.3 °C per decade.
