      <title># IncidentMind: An AI Incident Response Agent That Learns Using Hindsight</title>

  INTRODUCTION
&lt;/h2&gt;

&lt;p&gt;Production incidents are an unavoidable part of software systems.&lt;br&gt;
When an API starts returning errors, a database becomes unavailable, or a service suddenly slows down, engineers need to quickly understand what happened and decide what to do next.&lt;/p&gt;

&lt;p&gt;One problem is that organizations often have a lot of useful knowledge from previous incidents, but that knowledge may not always be easy to find or reuse when a new incident occurs.&lt;/p&gt;

&lt;p&gt;For the Hindsight AI Agents Hackathon, I built IncidentMind, an AI-powered incident response agent that uses memory to learn from previous incidents and use those experiences when handling new ones.&lt;/p&gt;

  THE PROBLEM
&lt;/h2&gt;

&lt;p&gt;Imagine that a company's Payment API suddenly starts returning HTTP 503 errors.&lt;/p&gt;

&lt;p&gt;An engineer investigating the problem might need to:&lt;/p&gt;

&lt;p&gt;. Understand the error.&lt;br&gt;
. Analyze the logs.&lt;br&gt;
. Identify possible causes.&lt;br&gt;
. Search through previous incidents.&lt;br&gt;
. Find out whether something similar happened before.&lt;br&gt;
. Determine how the previous incident was resolved.&lt;br&gt;
. Document the current incident after it is fixed.&lt;/p&gt;

&lt;p&gt;If a similar incident has already happened in the past, that previous experience can be extremely useful.&lt;/p&gt;

&lt;p&gt;But manually searching through old incident reports takes time.&lt;br&gt;
This is where I wanted to explore the idea of giving an AI agent memory of previous incidents.&lt;/p&gt;

  INTRODUCING INCIDENTMIND
&lt;/h2&gt;

&lt;p&gt;IncidentMind is an AI Incident Response Agent designed to analyze incidents and use previous incident experiences to assist with resolution.&lt;/p&gt;

&lt;p&gt;Instead of treating every incident as a completely new problem, the agent follows a workflow where previous experiences can become part of the reasoning context.&lt;/p&gt;

&lt;p&gt;The overall workflow is:&lt;/p&gt;

&lt;p&gt;Report Incident → Analyze → Recall Similar Incidents → Recommend Resolution → Resolve → Generate Postmortem → Remember&lt;/p&gt;

  WORKING
&lt;/h2&gt;

&lt;p&gt;IncidentMind follows a simple workflow:&lt;/p&gt;

&lt;p&gt;Report Incident → Analyze → Recall Similar Incidents → Recommend Resolution → Resolve → Generate Postmortem → Remember&lt;/p&gt;

&lt;p&gt;The agent first analyzes the reported incident, then searches its memory for relevant previous incidents. Based on the available information and past experiences, it provides a resolution recommendation. After the incident is resolved, the experience can be stored back into memory.&lt;/p&gt;

  HINDSIGHT MEMORY
&lt;/h2&gt;

&lt;p&gt;Hindsight is the core memory component of IncidentMind.&lt;br&gt;
It allows the agent to retain information about previous incidents and recall relevant experiences when a new incident occurs. This helps the agent use past incident knowledge instead of treating every new incident independently.&lt;/p&gt;

&lt;p&gt;Consider a previous incident where the Payment API returned 503 errors because of database connection pool exhaustion.&lt;br&gt;
Later, a similar Payment API incident occurs with the same type of error.&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;&lt;p&gt;IncidentMind can recall the previous incident and use its resolution as context when suggesting what to investigate in the new incident.&lt;/p&gt;&lt;/li&gt;
&lt;li&gt;&lt;p&gt;After an incident is resolved, IncidentMind can generate a postmortem containing the incident details, root cause, resolution, and outcome.&lt;/p&gt;&lt;/li&gt;
&lt;li&gt;&lt;p&gt;The resolved incident can then be retained as a new memory.&lt;br&gt;
This creates a continuous cycle where past incidents become useful experiences for future incidents.&lt;/p&gt;&lt;/li&gt;
&lt;/ul&gt;

  FUTURE SCOPE
&lt;/h2&gt;

&lt;p&gt;After an incident is resolved, IncidentMind can generate a postmortem containing the incident details, root cause, resolution, and outcome.&lt;br&gt;
The resolved incident can then be retained as a new memory.&lt;br&gt;
This creates a continuous cycle where past incidents become useful experiences for future incidents.&lt;/p&gt;

&lt;p&gt;IncidentMind demonstrates how an AI agent can use memory and past experiences to support incident response.&lt;/p&gt;

&lt;p&gt;Instead of starting from scratch with every new incident, the agent can recall what happened before, use that experience as context, and retain new experiences for future use.&lt;/p&gt;
