When Machines Fail, Experience Shouldn't Be Forgotten: The Story Behind RecallOps

By Vasundhara S | Hack with Bangalore 4.0

A Problem That Made Me Think Differently

Imagine working in a factory where hundreds of machines operate every day.

One morning, an important machine suddenly stops working. The maintenance team spends hours inspecting components, identifying possible causes, and trying different repairs.

Eventually, they solve the problem.

But what happens to everything they learned during those hours?

The successful repair might be recorded somewhere. The unsuccessful attempts might be forgotten. And when another machine develops a similar problem, a different technician could end up repeating the same troubleshooting process.

This made me think about something important.

What if the experience gained from every machine failure could help someone solve the next one?

That question became the starting point for RecallOps, our project for Hack with Bangalore 4.0.

We wanted to explore how artificial intelligence could help maintenance teams preserve useful knowledge instead of repeatedly starting from zero.

Why Machine Maintenance Needs More Than a Chatbot

Today, AI assistants can answer questions about almost anything.

If someone asks an AI chatbot why an industrial motor is overheating, it might suggest checking ventilation, bearings, lubrication, or electrical components.

These suggestions can be useful.

But there is an important limitation.

The chatbot may not know that a technician working in the same factory already encountered a similar problem last week.

It may not know that replacing a particular component failed, or that another repair successfully resolved the issue.

In industrial maintenance, that missing information matters.

Two machines can show similar symptoms while having different underlying problems. Previous repair outcomes provide additional context that general troubleshooting advice cannot always offer.

We wanted RecallOps to combine AI assistance with real maintenance experience.

Instead of asking only, "What could be wrong with this machine?" we wanted the application to consider another question:

"Have we faced something similar before, and what happened when we tried to fix it?"

Introducing RecallOps

RecallOps is an AI-powered industrial maintenance assistant designed to help technicians investigate machine problems using previous repair experiences.

The application allows users to report a machine failure, receive an AI-assisted investigation, review relevant historical cases, and record the repair performed.

After the repair, the technician can verify whether the problem was actually resolved.

That outcome becomes valuable information for future investigations.

Our project uses Hindsight Cloud to retain and retrieve relevant maintenance experiences.

This makes RecallOps different from a simple incident tracker.

An incident tracker tells us what happened.

RecallOps goes further by exploring how previous outcomes can influence the recommendations given during a new investigation.

The goal is to support technicians, not replace their knowledge or judgment.

A Realistic Example: The Cost of Repeating a Mistake

Consider a factory where a motor repeatedly overheats.

During the first breakdown, a technician assumes the motor has a bearing problem and replaces the bearings.

Unfortunately, the temperature remains unusually high.

The repair was unsuccessful.

Later, another technician discovers that restricted airflow is contributing to overheating. After cleaning the ventilation system, the motor temperature returns to an acceptable level.

The successful outcome is verified.

Now imagine a similar motor starts overheating again.

Without access to previous experiences, a technician might immediately suspect the bearings.

With RecallOps, the AI can retrieve the earlier incidents and highlight that bearing replacement did not resolve one case, while improving ventilation helped in another.

It can suggest checking airflow and cooling conditions as part of the investigation.

This does not guarantee that ventilation is the cause of every overheating problem.

However, it gives the technician useful historical context before choosing the next diagnostic step.

The important lesson is simple:

A failed repair can still be valuable if it prevents someone from repeating the same mistake.

Designing RecallOps Around the Technician

While developing our project, we wanted the application to feel like a practical maintenance workspace rather than a complicated AI demonstration.

A technician dealing with a machine failure needs information that is easy to find and understand.

That's why RecallOps is organized into six main sections.

Overview — Understanding What's Happening

The Overview page acts as the main dashboard.

It helps users access maintenance activity and begin investigating a new machine problem.

Instead of searching through several different tools, the technician can start from one central interface.

AI Investigations — Understanding the Problem

This is where the technician can review the AI-generated investigation.

RecallOps presents possible causes, recommended diagnostic steps, safety warnings, and relevant historical experiences.

We wanted users to see more than a single answer.

The historical evidence is important because it helps explain why certain checks may deserve attention.

Hindsight Memory — Remembering Experience

The Hindsight Memory section represents the central idea behind our project.

It allows users to explore previously retained maintenance experiences, including repair outcomes.

Successful and unsuccessful cases are distinguished so that past attempts are not automatically treated as proven solutions.

This helps make the agent's use of experience more understandable.

Incidents — Tracking Machine Failures

The Incidents section organizes reported problems and their progress.

A technician can review incident information and follow the maintenance workflow from reporting through investigation and verification.

This provides a clearer picture of the work performed.

Machines — Viewing Equipment History

The Machines section provides a machine-focused view of maintenance activity.

Users can explore equipment information and related incidents.

This helps connect individual problems with the machines they affect.

Learning Benchmark — Showing the Difference Memory Makes

The Learning Benchmark section demonstrates how an AI agent's recommendations can change when relevant historical experience is available.

Instead of only claiming that memory helps, we wanted to show a controlled comparison.

Together, these sections make RecallOps a complete demonstration of a memory-assisted maintenance workflow.

Why Verifying Repairs Is So Important

One of the most important decisions in our project was to distinguish between performing a repair and confirming that it worked.

Suppose a technician replaces a component and closes the incident.

If the application automatically assumes the repair was successful, the AI might later recommend the same action to someone else.

That would be misleading.

In RecallOps, the technician can record the repair action and then verify the outcome.

The verification stage helps distinguish successful repairs from unsuccessful attempts.

Only after the outcome is recorded can that experience be treated appropriately in future investigations.

This is especially important because AI recommendations depend on the quality of the information available.

An agent that remembers incorrect outcomes could repeat incorrect assumptions.

By emphasizing verification, we wanted to make the learning process more responsible and meaningful.

How Hindsight Helps Our Application Learn

Hindsight is the long-term memory component behind RecallOps.

When a technician begins a new investigation, the application can retrieve relevant experiences from previous incidents.

These experiences are then provided as context to the AI model.

The model can consider what worked previously, what failed, and how those cases relate to the current symptoms.

After a new repair outcome is verified, that experience can also be retained for future use.

This creates a cycle in which maintenance knowledge is continuously collected and reused.

Importantly, the language model is not retrained after every repair.

Instead, the agent uses retrieved memories to make its responses more informed.

To me, this is one of the most interesting possibilities of memory-enabled AI agents.

Their usefulness can come not only from general knowledge but also from relevant experiences accumulated through actual workflows.

Demonstrating the Value of Memory

We wanted to make the difference between ordinary AI reasoning and memory-informed reasoning visible.

For this, our team created a small controlled synthetic experiment.

The experiment compared a baseline condition without an additional verified repair experience against a condition that included relevant Hindsight memory.

Across three predefined scenarios, the baseline achieved 0 out of 3 target-aligned diagnostic prioritizations, while the memory-informed condition achieved 3 out of 3.

This result illustrates how historical information can influence an agent's recommendations.

However, the experiment uses synthetic scenarios and a small sample. It does not prove that RecallOps has 100% diagnostic accuracy or that it will always outperform other approaches in real factories.

A larger evaluation using actual maintenance records would be needed to measure real-world effectiveness.

For our prototype, the benchmark serves as a clear demonstration of how remembered experience can change AI-assisted decision-making.

Turning Our Idea into a Working Application

RecallOps is not just a design concept.

Our team developed a working web application using React, Vite, and Tailwind CSS for the interface.

The backend uses Python and FastAPI to manage incidents, investigations, repair actions, and outcome verification.

Groq provides AI inference, while Hindsight Cloud handles long-term memory.

We deployed the frontend using Vercel and the backend using Render.

We also performed automated testing of the connected application.

Our reported quality-assurance audit completed 56 automated checks successfully, covering browser interactions, backend APIs, integration workflows, and other reliability checks.

A complete browser-based demonstration was able to create an incident, generate an AI investigation, retrieve historical precedents, record a repair, and verify the outcome.

This was an important milestone because it showed that our main workflow could operate across the deployed services.

What We Learned While Building RecallOps

Working on this project changed how I think about AI applications.

A useful AI product is not just about generating impressive answers.

It is also about understanding the user's actual problem and presenting information in a way that supports better decisions.

For maintenance technicians, that means keeping the workflow clear, showing relevant evidence, and distinguishing suggestions from verified results.

We also learned that historical information is only useful when its context is preserved.

A failed repair should not be presented as a successful solution, and a successful repair for one machine should not automatically be considered correct for every similar machine.

These distinctions matter when building systems that people may rely on.

Limitations and Future Possibilities

RecallOps is currently a hackathon prototype.

Its maintenance examples and controlled learning benchmark are based on demonstration scenarios rather than a large real-world industrial dataset.

The system also depends on accurate information being entered by technicians.

Although Hindsight stores retained experiences in the cloud, the prototype's SQLite incident database runs in an environment where local records may not survive instance replacement or redeployment.

Before real industrial deployment, we would need stronger authentication, durable operational storage, broader evaluation, and more extensive safety validation.

In the future, we would like to explore live sensor integration, automatic equipment monitoring, organization-specific maintenance knowledge, and more detailed technician collaboration.

These improvements could make RecallOps more useful in practical industrial environments.

Final Thoughts: Every Failure Should Teach Us Something

The idea behind RecallOps is bigger than simply creating another AI chatbot.

We wanted to explore what happens when an AI assistant can use the experience gained from previous work.

A machine failure may interrupt production, but the investigation also creates knowledge.

If that knowledge is preserved, it may help someone solve a similar problem more efficiently in the future.

That's the purpose of RecallOps.

Machines may fail more than once. But maintenance teams shouldn't have to forget what they learned the first time.
