This paper aims at analyzing the trend of national budget allocation in Iran over the last 100years to evaluate the focus of the Iranian state on the four phases of Preparedness, Mitigation, Response, and Recovery and propose modifications. &lt;/p&gt;

&lt;p&gt;All 6336 special community pharmacies participating in the NHI (National Health Insurance) served as mask-selling sites. Access to masks by citizens was determined and controlled, based on the weekly rationing of the number of purchasable masks per citizen and the last digit of their NHI card number. Masks were available on different weekdays for holders of cards ending with odd and even numbers, except on Sundays, when everyone was eligible to buy a mask. &lt;/p&gt;

&lt;p&gt;Implementing the program has provided equal access to masks for all citizens across Taiwan. It has stabilized the pricing of masks and mitigated the public's anxiety of a perceived likely market shortage. &lt;/p&gt;

 To compare costs and outcomes following knee chondroplasty with Coblation versus mechanical shaver debridement (MSD) in patients with grade III articular cartilage lesions of the knee. &lt;/p&gt;

&lt;p&gt;A decision-analytic model was developed to compare costs and outcomes of the two methods from a US payer perspective. We used published clinical data from a single-center randomized clinical trial (RCT) designed to compare outcomes between Coblation and MSD in patients with grade III articular cartilage lesions of the medial femoral condyle. Following primary knee chondroplasty, patients experienced either treatment success (no additional surgery required) or required a revision over the 4year follow-up period. Costs associated with the initial chondroplasty, physical therapy sessions through the 6week postoperative period, and revision rates at 4years post-surgery were estimated using 2018 US Medicare Physician Fee Schedule. Sensitivity analyses including a 10year time horizon and threshold analyses were performed to test the robustness of the model. &lt;/p&gt;

&lt;p&gt;The estimated total cost per patient was $4614 and $7886 for Coblation and MSD, respectively, resulting in cost-savings of $3272 in favor of Coblation, making it a dominant strategy because of lower costs and improved clinical outcomes. Threshold analysis showed that Coblation remained dominant even when revision rates were assumed to increase from the base case rate of 14-66%. Sensitivity analyses showed that cost-saving results were insensitive to variations in revision rates, number of physical therapy sessions and the time horizon used. &lt;/p&gt;

 This study aimed to investigate the predictive value of the D-dimer level for venous thromboembolism (VTE) events during puerperium of women age at 35 years or older, as well as to identify other risk factors associated with the occurrence of VTE. &lt;/p&gt;

&lt;p&gt;It was a prospective observational cohort study, from January 2014 to December 2018, which involved 12,451 women age 35 or older who delivered at least 28 weeks of gestation at Women's Hospital of Zhejiang University, School of Medicine. The maternal and fetal demographic characteristics, pregnancy complications, imaging finding and results of laboratory test within postpartum 24 h including D-dimer level, platelet counts and fibrinogen level were collected for analyses. &lt;/p&gt;

&lt;p&gt;30(2.4‰) women were identified as VTE, including 1 pulmonary embolism event and 29 deep venous thrombosis events. The receiver operating characteristic (ROC) curve analysis suggested the best cutoff point for D-dimer level within postpartum 24 h of women age 35 or older was 5.545 mg/L, with a specificity of 70.0% and a sensitivity of 75.4%. Besides, there was no statistical correlation between platelet counts and VTE, as well as between fibrinogen level and VTE. On multivariate analysis, D-dimer≥5.50 mg/L (OR = 5.874, 95%CI 2.678-12.886) and emergency cesarean section (OR = 11.965, 95%CI 2.732-52.401) were independently associated with VTE in puerperium of women age 35 or older. &lt;/p&gt;

      <title>Age-Associated Neural Issues involving COVID-19: A Systematic Review and also Meta-Analysis.</title>

      <description>&lt;p&gt;Symphytum officinale (comfrey), Tussilago farfara (coltsfoot) and Borago officinalis (borage) have long histories of therapeutic use, but their safety has been questioned due to the presence of unsaturated pyrrolizidine alkaloids (PAs). The evidence base underlying these concerns relies in part on case reports. This systematic review assesses these case reports for their reliability to inform this debate. METHOD Study selection was restricted to case reports describing possible pyrrolizidine alkaloid related harm and ingestion of comfrey, coltsfoot or borage. An extensive search of academic databases was conducted. Papers meeting the criteria were critically appraised. RESULTS The search resulted in 11 appropriate case reports, none of which involved borage. Nine reports were assessed for causality and indicated some degree of association between the material ingested and the adverse event. Lack of unequivocal identification of the species ingested compromised attribution and was a significant source of uncertainty. Three levels of identity confusions were found; misidentification or substitution at the level of the whole herb; omission of appropriate botanical identification and attribution of a specific PA to either comfrey or coltsfoot when it is a constituent found in other plants of established toxicity. CONCLUSION These cases are an unreliable body of evidence on which to draw conclusions about the safety of the oral consumption of Symphytum officinale and Tussilago farfara. Toxicological studies based on oral ingestion of phytochemically-complex preparations of these herbs may be the most accurate methodology for assessing clinical risk. The development of a new drug active substance is not only time-consuming and expensive, but also a chain of operations that often fails. However, increasing the bioavailability, effectiveness, safety, or targeting the drugs used in clinic by various methods, such as nanoparticles (NPs), may be a more effective way of using them in clinic. In addition, NP formulations are becoming increasingly popular in modern medical treatments. Angiogenesis, formation of new capillaries from a pre-existing one, fundamentally occurs in physiological processes such as wound healing, embryogenesis and menstrual cycle, also has a vital role in pathology of cancer, psoriasis, diabetic retinopathy and chronic inflammation. The Hen's Egg Test on the Chorioallantoic Membrane (HET-CAM) assay is a useful, well established and animal alternative in vivo procedure for evaluation of anti-inflammatory potentials and anti-irritant properties of nano drug delivery systems. In this study, diclofenac sodium (DS) loaded PLGA NPs were prepare results indicated that NP-1 coded NP formulation showed significantly good anti-inflammatory potential at low dose. As a result, a low dose high anti-inflammatory effect was achieved with the NP structure of DS. To the best of our knowledge this is the first study on in vivo anti-inflammatory activities of DS loaded PLGA NPs by HET-CAM. Q fever is considered one of the most important zoonoses in Australia. Whilst ruminants are the primary reservoirs for Coxiella burnetii, and the major source of human infection, human cases have also been reported following contact with pet dogs and cats. This study aimed to estimate the prevalence of seropositivity to, and bacterial shedding of, C. burnetii by pet dogs and cats in a region with a high human Q fever incidence and explore risk factors for C. burnetii exposure. Samples (serum, whole blood, reproductive tissue, reproductive swabs) and questionnaires (completed by the pet's owner) were collected from dogs and cats from eight communities across remote New South Wales (NSW), Australia. Overall 86/330 dogs (26.1%, 95% CI 21.3-30.8%) and 19/145 cats (13.1%, 95% CI 7.6-18.6%) were seropositive to C. burnetii. Seroprevalence varied significantly between communities and was highest in communities within 150 km of a 2015 human Q fever outbreak. Feeding raw kangaroo was identified as a risk factor for seropositivity (adjusted OR 3.37, 95% CI 1.21-9.43). Coxiella burnetii DNA was not detected from any dog or cat whole blood, reproductive tissue or vaginal/preputial swab using qPCR targeting the IS1111 and com1 genes. Our findings suggest that companion animals are frequently exposed to C. burnetii in western NSW. Geographical variation in C. burnetii seroprevalence amongst companion animals - which corresponds with a human Q fever outbreak - suggests a shared environmental source of infection is likely with important consequences for public and animal health. The lack of detection of C. burnetii DNA from healthy companion animals suggests that pet dogs and cats are not an important reservoir for human Q fever infection outside a narrow periparturient window. We discuss relevant pathology and surgical treatment through a review of previous literature.A 39-year-old man had received boron neutron capture therapy (BNCT) for a grade II astrocytoma (compatible with diffuse astrocytoma, not otherwise specified in the WHO 2016 criteria). He returned to his previous work after surgery, but, 7 years later, he suddenly developed seizures, and his health condition deteriorated. Therefore, he underwent a second surgery. The mass removed in the second operation was mostly necrotic as a result of previous radiation treatment. He then showed no signs of recurrence and did not require any treatment other than antiepileptic drugs for 25 years. He was able to be employed by a listed company until the age of 65 years for light jobs as a physically handicapped individual. This case suggests the effectiveness of BNCT even for rather low-grade astrocytomas.A 6-year-old female was incidentally found to have a brain tumor. Magnetic resonance imaging (MRI) demonstrated a gadolinium-enhanced mass in the left parietal lobe. We performed gross total resection with the assistance of fluorescent guidance by 5-aminolevulinic acid (5-ALA). A histological examination of the tumor specimen showed well-differentiated astroblastic features with focal anaplasia. Fluorescence in situ hybridization (FISH) revealed meningioma 1 (MN1) gene alteration and supported our diagnosis. She received local radiotherapy and oral temozolomide followed by maintenance temozolomide chemotherapy, and the tumor was well controlled without any neurological deficit for 27 months. Our case is considered to be valuable since it describes a patient who is diagnosed to have a well-differentiated astroblastoma with both focal anaplastic features and MN1 gene rearrangement. A larger study is warranted to establish evidence supporting the diagnosis and treatment of astroblastoma with molecular characteristic features. MN1 alteration will be a diagnostic marker for astroblastoma in the future.Isolated hypoglossal nerve palsy (IHP), or hypoglossal nerve palsy without any other neurological signs, is rare. We report a woman with atlantoaxial dislocation (AAD) who presented with IHP due to hypoglossal nerve compression by an osteophyte at the hypoglossal canal. A 77-year-old woman presented with speech difficulties and the feeling of a swollen tongue on the left side for 3 days. Her only neurological feature was left hypoglossal nerve palsy. CT and high-resolution MRI revealed this rare mechanism of IHP.We describe a rare case of 42-year-old female who had possible double crush syndrome caused by iatrogenic spinal epidermoid tumor (ET) associated with lumbar puncture as well as concomitant sacral Tarlov cyst in close proximity. She presented with progressive left-sided perianal pain. She had a history of a Caesarean section with lumbar spinal anesthesia. Magnetic resonance imaging (MRI) demonstrated a relatively small intradural extramedullary solid lesion at L5/S1 level and cystic lesion at S2 level. We considered there were two different lesions, such as a tumor and Tarlov cyst; however, we could not rule out the possibility of a single lesion with two different components. Furthermore, there was a distinct compression at more than one locations along the course of the left S2 nerve root and we suspected possible double crush syndrome. We conducted tumor removal and the lesion turned out to be two different pathologies, such as an ET and Tarlov cyst. Both lesions were intraopertively pinching the left S2 nerve root at different sites as expected. The tumor was successfully removed and the cyst wall was imbricated and sutured. We need to take the possibility of ET into consideration if the patient underwent invasive spinal procedure previously. We also have to pay attention to the possibility of double crush syndrome if the nerve root possibly holding the responsibility for symptoms is compressed at two or more sites. This is the first report of possible double crush syndrome caused by acquired spinal tumor and congenital Tarlov cyst.Extramammary Paget's disease (EMPD) is a rare form of neoplasm. Metastasis of EMPD to locations other than lymph nodes and intra-epithelial regions is rare; there are a limited number of case reports of metastases to the liver, lung, bone, and brain. We present a rare case of EMPD that metastasized to the brain and was treated with surgical resection. A 66-year-old man presented with a small palpable mass in the scrotum. After 5 years of observation, he was diagnosed with EMPD that metastasized to the lymph nodes and lung. Tumor resection and postoperative chemotherapy were performed. Six months after the last chemotherapy treatment, he presented with a right temporal lobe tumor and underwent surgical resection. Histopathological analysis revealed brain metastasis of EMPD. Three months after surgery, magnetic resonance imaging (MRI) showed local tumor recurrence, and intensity modulated radiation therapy (IMRT) (45 Gy/15 Fr) was performed. Although the metastatic brain tumor was well controlled, the primary tumor progressed. He was provided best supportive care and died 5 months after brain tumor resection. In this report, we present a rare case of brain metastasis of EMPD, treated with surgical resection, and histopathologically confirmed to be metastatic EMPD.Fenestration of the supraclinoid internal carotid artery (ICA) is extremely rare and may occasionally be associated with saccular aneurysms; nevertheless, the natural history remains unclear. The authors reviewed the cases of fenestration of the supraclinoid ICA and evaluated the clinical characteristics and angioarchitecture with particular focus on the incidence for aneurysm development and rupture. Previously reported 24 cases and 1 new case from our institute were examined for the presence of aneurysms (including size and presence of bleb) leading to subsequent subarachnoid hemorrhage (SAH). There are different treatment approaches for these conditions, such as initial treatment of the aneurysm before revascularization of the stenosis; single-stage endovascular treatment of both lesions; stenosis eliminating followed by treatment of the aneurysm, or without treating the aneurysm. But, taking into account the risk of aneurysm rupture on the one hand and the risk of ischemic stroke on the other, it is sometimes difficult to choose the right management strategy. The article describes a clinical case of endovascular treatment of multiple extracranial stenosis and coexistent unruptured wide-neck aneurysm of the middle cerebral artery. The endovascular treatment of the carotid stenosis and aneurysm using a woven endobridge device was performed in one session; endovascular treatment of vertebral artery stenosis with Szabo technique was performed in another session.A left partial anomalous vein connection and connection between left upper pulmonary vein and left internal thoracic vein was discovered in a 71-year-old man, with no history of previous cardiac surgery or an atrial septal defect, underwent a thorax HRCT scan without contrast media, following a chest x-ray, due to repeated bouts of smoking-related bronchitis. Left partial anomalous pulmonary vein connection is a rare venous variant, which more commonly affects the right side, resulting in a left to right shunt [9].Proximal tibiofibular joint (PTFJ) pathology is an uncommon but perhaps underappreciated cause of lateral knee pain. While imaging guided therapeutic injections for diagnosis and management of joint related symptoms are now commonplace and numerous techniques for accessing most joints in the body have been documented, a technique for fluoroscopically guided injection of the PTFJ has not been yet described in the literature. We present a case of an adult patient who presented with lateral knee pain refractory to conservative management who opted for a fluoroscopically-guided therapeutic injection of her PTFJ.The current study aimed to determine relationships between oculomotor behavior and aesthetical evaluation of paintings. We hypothesized that paintings evaluated as beautiful compared to nonbeautiful would be associated with different oculomotor behavior in terms of fixation duration, viewing time, and temporal and spatial distribution of attention. To verify these hypotheses, we examined forty participants that looked at and evaluated 140 figurative paintings while their eye movements were recorded. To analyze data, we used divergence point analysis (DPA) and recurrence quantification analysis (RQA). The results of the DPA suggested that fixation durations longer than 229 ms are sensitive to the effect of aesthetical evaluation. We also found that the effect of aesthetical evaluation was significant in the time window between 2.3 s and 19.8 s of viewing a painting. The results of the RQA suggested that the participants viewed paintings evaluated as beautiful in a more structured manner compared to those evaluated as nonbeautiful, which suggests higher involvement of top-down processes while facing beautiful artwork. We discuss and refer these results to the literature on cognitive processes related to aesthetical evaluation of paintings. &lt;br&gt;
 Interleukin (IL)-34 acts as an alternative ligand for the colony-stimulating factor-1 receptor and controls the biology of myeloid cells, including survival, proliferation, and differentiation. IL-34 has been reported to be expressed in cancer cells and to promote tumor progression and metastasis of certain cancers via the promotion of angiogenesis and immunosuppressive macrophage differentiation. We have shown in our previous reports that targeting IL-34 in chemo-resistant tumors in vitro resulted in a remarkable inhibition of tumor growth. Also, we reported poor prognosis in patients with IL-34-expressing tumor. Therefore, blocking of IL-34 is considered as a promising therapeutic strategy to suppress tumor progression.       <description>&lt;p&gt;Vector-born disease models are extensively used for surveillance and control processes. The most simple and generally use model (SEIR-SEI model) cannot explain a variety of phenomena involved in these diseases spread and development. In order to obtain a wider insight of the vector-born disease models (and the dynamics involved in them), this work focuses into analyse the classical model, a modified versions of it, and 8 their parameters. The modified version includes host mobility, 9 environmental, re-susceptibility, and mosquito life cycle considerations. As results it is observed that there are a limiting number of parameters that play the most important roles in the dynamics (those related to mortality rates, recovery rate from infectious, and pathogen transmission probabilities). Therefore, parameters determination should focus primarily into estimate these values. Stronger effects of the environmental variables are observed and expected by using different parameters and/or the use of multiple environmental variable at the same time.This study evaluated the effect of row spacing on growth, yield and yield components of common bean varieties at Jimma. The objectives of the study were (i) to evaluate the best performed common bean variety (ii) to determine the optimum row spacing for different common bean varieties under the test environment. The experiment was laid out in randomized complete block design (RCBD) in factorial arrangement with three replications. A combination of three common bean Varieties (Nasir, Goberesha and Local Asendabo) and row spacing at four levels (30, 40, 50, 60cm) were used as experimental treatments. Leaf number was significantly varied between the varieties; whereas row spacing significantly influence leaf area index. The main effect of row spacing and the interaction effect did not show significant influence on both. Hundred seed weight and number of seed per pod were varied between the varieties and row spacing. Hundred seed weight was increased with row spacing. Pod number per plant, nodule number per plant, grain yield and harvest index was significantly affected row spacing, variety and their interactions. Grain yields increased with plant population assuming a parabolic curve to the plant density of both determinate varieties at 40cm row spacing. The highest yield for variety-Nasir and variety-Goberesha were observed at40 cm row spacing; whereas the highest yield for the local variety was recorded at 50 cm row spacing.The analysis of the distribution of microfractures and micropores is important to accurately characterise mass transfer within a rock body. In this paper, a new 'simultaneous polarization-fluorescence microscopy' method is presented, which can be used to analyse the distribution of microscopic voids, including microfractures and micropores, in granitic rock. In this method, thin sections prepared with fluorescent dye are analysed under a polarizing microscope equipped with a fluorescent reflected light source. Using both the transmitted and the fluorescent light sources, both the distribution of microfractures and micropores, and petrographic characteristics (mineral occurrences) can be determined efficiently and simultaneously. The distribution of microfractures and micropores observed in images of granites obtained using simultaneous polarization-fluorescence microscopy is consistent with the distribution observed in backscattered electron images. The low magnification characterisation of the distribution of microscopic voids also provides targeting for subsequent studies including scanning electron microscopy under high magnification, chemical analysis, and image processing. &lt;br&gt;
 Adherence to treatment guidelines and treatment success are low in Type 2 diabetes mellitus (T2DM). This study aims to capture the physician perspective on T2DM guideline adherence and identify levers for increasing adherence. &lt;/p&gt;

&lt;p&gt;A survey among German physicians captured the perceived value of 4 areas in the national treatment guideline (NVL), 13 possible barriers, and 9 possible enablers for guideline adherence. Perceived value was assessed by ranking 4 NVL areas by implementation difficulty and impact on treatment success. Barriers and enablers were assessed by rating their influence on guideline deviation and adherence. The consistency of results across subgroups was assessed using Fisher's exact test. &lt;/p&gt;

&lt;p&gt;Responses from 46 physicians showed a strong consensus about the value of each NVL area. Physicians perceived patient inability and demotivation to be the strongest adherence barriers (93%, 78%). All queried enablers were approved by ≥ 50% of participants.       <description>&lt;p&gt;A highly stereoselective, one-pot, multicomponent method has been developed to synthesize pyrrolizidine- and N-methyl pyrrolidine-substituted spirooxindole derivatives. The [3 + 2] cycloaddition reaction involves the reaction between the dipole azomethine ylides, generated in situ from the reaction between isatin and secondary amino acids such as L-proline or sarcosine, and α,β-unsaturated carbonyl compounds as the dipolarophile. The reaction condition was optimized to achieve excellent regio- and stereoselectivity. Products were obtained in good yield using ethanol as a solvent at the reflux temperature. Scanning electron microscopy, X-ray diffraction, Fourier transform infrared spectroscopy, and X-ray photoelectron spectroscopy were used to characterize the properties of three kinds of carbon. Kinetic and thermodynamic experiments were performed. The results showed that the experimental data were fitted well by the Freundlich model and the pseudo-second-order kinetic model, and the adsorptions on the three kinds of carbon were all spontaneous, endothermic processes. The adsorption of Cr6+ by biochar was in accordance with a chemisorption process. The adsorption contribution rate of the OC was 97%, which was much higher than that of the IC. Electrostatic attraction and redox reaction were the main mechanisms of adsorption, and among them, the contribution rate of the redox reaction accounted for 61.49%. The reduced Cr3+ could both exchange ions with K+ and dissociate into solution by electrostatic repulsion; the amount of Cr3+ released into the solution was approximately 17.07 mg/g, and the amount of Cr3+ ions exchanged with K+ was 0.29 mg/g. These results further elucidate the adsorption mechanism of Cr6+ by biochar.In the present work, we report for the first time an in-depth study of the factors influencing porous cellulose film structure formation during the nonsolvent-induced phase separation (NIPS) process from biopolymer solutions in ionic liquid-based solvents. The length of the alkyl chain of the ionic liquid's cation, the solvent/co-solvent ratio, and the type of the cellulose precursor used were found to have great influence both on cellulose solution formation and properties and to the NIPS process with water acting as nonsolvent. In the undiluted form, both studied ionic liquids proved to dissolve almost equally well the cellulose; however, due to differences in viscosities of the formed biopolymer solutions and due to differences in miscibility with water of the two ionic liquids, the used ionic liquid had a strong influence on the film's porous structure formation. The use of increasing amounts of an aprotic co-solvent, here dimethylsulfoxide, improved biopolymer solubilization and also led to the formation of a more pronounced macroporous structure during the NIPS process. The cellulose type also affected the porous structure generation during the NIPS process with the increase of the molecular weight of the precursor, the viscosity of the formed biopolymer solution increased and the tendency to generate macroporous structures decreased.Mitochondrial metabolism plays an essential role in various biological processes of cancer cells. Herein, we established an experimental procedure for the metabolic assessment of mitochondria in cancer cells. We examined procedures for mitochondrial isolation coupled with various mitochondrial extraction buffers in three major cancer cell lines (PANC1, A549, and MDA-MB-231) and identified a potentially optimal and generalized approach. The purity of the mitochondrial fraction isolated by the selected protocol was verified using specific protein markers of cellular components, and the ultrastructure of the isolated mitochondria was also analyzed by transmission electron microscopy. The isolation procedure, involving a bead beater for cell lysis, a modified sucrose buffer, and differential centrifugation, appeared to be a suitable method for the extraction of mitochondria from cancer cells. Electron micrographs indicated an intact two-layer membrane and inner structures of mitochondria isolated by this procedure. Metabolomic and lipidomic analyses were conducted to examine the metabolic phenotypes of the mitochondria-enriched fractions and associated bulk cancer cells. A total of 44 metabolites, including malate and succinate, occurred at significantly higher levels in the mitochondrial fractions, whereas 51 metabolites, including citrate, oxaloacetate, and fumarate of the Krebs cycle and the oncometabolites glutamine and glutamate, were reduced in mitochondria compared to that in the corresponding bulk cells of PANC1. Similar patterns were observed in mitochondria and bulk cells of MDA-MB-231 and A549 cell lines. A clear difference between the lipid profiles of bulk PANC1, MDA-MB-231, and A549 and corresponding mitochondrial fractions of these cell lines was detected by principal component analysis. Here, we propose a 3D sensor for AD diagnosis using blood plasma, with pairs of 3D silicon micropillar electrodes with a comprehensive circuit configuration. The sensor is developed with synthesized artificial peptide and impedance analysis based on a maximum signal-to-noise ratio. Its sensitivity and selectivity were verified using an in vitro test based on samples of human blood serum, which showed its feasibility for application in diagnosis of AD by testing blood plasma of the AD patient. The 3D sensor is designed to improve reliability by checking the impedance of each pair multiple times via constructing a reference pair and a working pair on the same sensor.
