      <description>&lt;p&gt;BACKGROUND Prosthetic repair of inguinal hernias is one of the most performed surgical procedures. BACKGROUND It remains uncertain whether there is a benefit to perioperative beta-blocker use on outcomes after non-cardiac surgery. This meta-analysis aims to update the evidence regarding the associations between beta-blocker exposure and patient major short-term outcomes following non-cardiac surgery. METHODS Pubmed, Embase, and the Cochrane Central Register from their inception to May 2019 were searched by two independent authors. Observational studies reporting associations between perioperative beta-blocker treatment and short-term outcomes including 30-day all-cause mortality (ACM), 30-day major adverse cardiovascular events (MACE) and 30-day stroke risk were selected for inclusion. Meta-analyses were carried out by using random effects models. RESULTS Nineteen studies with a total of 1,711,766 participants were identified. Beta-blocker exposure was associated with reduced 30-day all-cause mortality (ACM) (RR 0.83, 95% CI 0.72 to 0.96). No evidence of publication bias was observed. Subgroup analyses revealed that significant 30-day survival benefits were observed in prospective, population-based studies, drug exposure period last till 1-2 months after surgery, patients having abdominal gastrointestinal surgery or having 3-4 cardiac risk factors. Beta-blocker exposure was associated with increased 30-day ACM among patients with no cardiac risk factors (RR 1.30, 95% CI 1.19 to 1.43). However, meta-analysis demonstrated a non-significant risk reduction in 30-day MACE (RR 1.03; 95% CI 0.85 to 1.25) or 30-day stroke risk (RR 0.86; 95% CI 0.44 to 1.68) with beta-blocker exposure. CONCLUSIONS The results of the current meta-analysis indicate beta-blocker exposure may be a significant indicator for 30-day ACM, but not for 30-day MACE or 30-day stroke risk. The association between beta-blocker exposure and long-term outcomes deserves further investigation. OBJECTIVES To investigate whether immediate weightbearing after intramedullary fixation of extra-articular distal tibial fractures could avoid secondary replacement. METHODS We prospectively included 167 patients receiving intramedullary nailing in treatment of distal tibial fractures. All these patients were encouraged to bear weight as tolerated postoperatively. One hundred and fifty-five patients who did not bear weight immediately after surgery were retrospectively included as historical control. RESULTS The mean immediate lateral and anterior DTA were 88.9 ± 3.9 and 85.2 ± 3.5° for historical control and 88.7 ± 3.6 and 84.9 ± 3.8° for immediate weight bearing group (lateral DTA P = 0.715; anterior DTA P = 0.734). The mean final lateral and anterior DTA were 88.1 ± 3.3 and 84.1 ± 4.3° for historical control and 87.9 ± 5.0 and 84.5 ± 5.1° for the immediate weightbearing group (lateral DTA P = 0.857; anterior DTA P = 0.788). Strikingly, the immediate weightbearing resulted in accelerated healing (3.5 ± 1.2 versus 4.9 ± 1.3 months, P = 0.023) and decreased nonunion rate (2.4% versus 7.1%, P = 0.027). The rates of infection and soft-tissue necrosis were similar between the two groups. CONCLUSIONS Immediate weightbearing after IMN fixation of extra-articular distal tibial fractures led to a similar change in alignment compared with our historical control without immediate weightbearing. Immediate weightbearing appears to be safe for most patients and might be able to accelerate fracture healing and decrease nonunion rate. OBJECTIVES Cardiac tumors and their associated outcomes are poorly characterized. This study sought to comprehensively assess the epidemiology and natural history of primary and secondary malignant cardiac tumors (PMCT and SMCT), a well as establish predictors of mortality. METHODS A comprehensive literature review was performed to identify articles reporting on PMCTs and SMCTs. New entities of Ewing-like tumors, with fusion transcripts that are related to the oncogenic EWSR1-FLI1 fusion seen in the majority of Ewing tumors, are being mapped. These tumors, although sharing much of the same morphologic features as classic Ewing sarcoma, behave differently and may require a different treatment. There are also controversies regarding local treatment of Ewing sarcoma. The radiosensitive nature of the disease and the tendency for Ewing sarcoma to arise in the axial skeleton make local treatment very challenging. Surgical treatment and radiotherapy have their pros and cons, which may give rise to different treatment strategies in different centers around the world. This review article discusses some of these controversies and reproduces the highlights from recent publications with regard to diagnostics, systemic treatment, and surgical treatment of Ewing sarcoma.The predictable nature of deoxyribonucleic acid (DNA) interactions enables assembly of DNA into almost any arbitrary shape with programmable features of nanometer precision. The recent progress of DNA nanotechnology has allowed production of an even wider gamut of possible shapes with high-yield and error-free assembly processes. see more Most of these structures are, however, limited in size to a nanometer scale. To overcome this limitation, a plethora of studies has been carried out to form larger structures using DNA assemblies as building blocks or tiles. Therefore, DNA tiles have become one of the most widely used building blocks for engineering large, intricate structures with nanometer precision. To create even larger assemblies with highly organized patterns, scientists have developed a variety of structural design principles and assembly methods. This review first summarizes currently available DNA tile toolboxes and the basic principles of lattice formation and hierarchical self-assembly using DNA tiles. Special emphasis is given to the forces involved in the assembly process in liquid-liquid and at solid-liquid interfaces, and how to master them to reach the optimum balance between the involved interactions for successful self-assembly. In addition, we focus on the recent approaches that have shown great potential for the controlled immobilization and positioning of DNA nanostructures on different surfaces. The ability to position DNA objects in a controllable manner on technologically relevant surfaces is one step forward towards the integration of DNA-based materials into nanoelectronic and sensor devices.Much attention has been focused lately on the Opportunistic Routing technique (OR) that can overcome the restrictions of the harsh underwater environment and the unique structures of the Underwater Sensor Networks (UWSNs). OR enhances the performance of the UWSNs in both packet delivery ratio and energy saving. In our work; we propose a new routing protocol; called Energy Efficient Depth-based Opportunistic Routing with Void Avoidance for UWSNs (EEDOR-VA), to address the void area problem. EEDOR-VA is a reactive OR protocol that uses a hop count discovery procedure to update the hop count of the intermediate nodes between the source and the destination to form forwarding sets. EEDOR-VA forwarding sets can be selected with less or greater depth than the packet holder (i.e., source or intermediate node). It efficiently prevents all void/trapped nodes from being part of the forwarding sets and data transmission procedure; thereby saving network resources and delivering data packets at the lowest possible cost. The results of our extensive simulation study indicate that the EEDOR-VA protocol outperforms other protocols in terms of packet delivery ratio and energy consumption.The PARP family consists of 17 members with diverse functions, including those related to cancer cells' viability. Several PARP inhibitors are of great interest as innovative anticancer drugs, but they have low selectivity towards distinct PARP family members and exert serious adverse effects. We describe a family-wide study of the nicotinamide (NA) binding site, an important functional region in the PARP structure, using comparative bioinformatic analysis and molecular modeling. Mutations in the NA site and D-loop mobility around the NA site were identified as factors that can guide the design of selective PARP inhibitors. The My Research Legacy study examined ideal cardiovascular health using the Life's Simple 7 survey and data from digital health devices. We hypothesized that digital devices provide a more objective view of overall cardiovascular health status than self-reported measures. Therefore, we analyzed weight and activity data recorded by digital devices to recalculate the Life's Simple 7 Health Score. All study participants (n = 1561) answered the survey, while a subgroup (n = 390) provided data from digital devices. Individuals with digital devices had a lower body mass index (BMI) and higher weekly minutes of vigorous exercise than participants without digital devices (p less then 0.01). Baseline Health Scores were higher in individuals with digital devices compared to those without (7.0 ± 1.6 vs. 6.6 ± 1.6, p less then 0.01). Data from digital devices reveal both increases and decreases in measured vs. self-reported BMI (p less then 0.04) and weekly minutes of moderate and vigorous exercise activity (p less then 0.01). Using these data, a significant difference was found between the recalculated and the self-reported Life's Simple 7 Health Score (p less then 0.05).       <description>&lt;p&gt;The isolation of autochthonous yeast species presents a good strategy to select new microorganisms for developing an adequate inoculum to carry out fermentations and generate representative products of the cider production zone. However, non-Saccharomyces yeasts have been considered to have low capacity to carry out a complete fermentation as Saccharomyces cerevisiae. In this work, five autochthonous yeasts from a cider fermentation process were isolated and identified as Saccharomyces cerevisiae, Kluyveromyces marxianus, Pichia membranaefaciens, P. kluyveri and Zygosaccharomyces rouxii. A series of fermentations were developed at laboratory level, using each species individually and it was observed that only S. cerevisiae was able to finish the process. K. marxianus consumed less than 50% of the sugars; P. kluyveri and Z. rouxii consumed less than 70% and P. membranaefaciens consumed more than 90% but the yield (ethanol produced for sugar consumed (YP/S)) was 0.39. see more Nevertheless, the addition of magnesium, zinc and nitrogen increased the fermentative capacity of almost all species K. marxianus, Z. rouxii and P. kluyveri, showed an increase in ethanol production when nutrients were added, obtaining more than 80 g/L of ethanol, and showing that those nutrients are necessary to complete the fermentation. This work describes the potential use of different non-Saccharomyces species to carry out fermentation of apple juice and highlights the importance of certain nutrients to enable an efficient alcoholic fermentation and the generation of desirable volatile compounds for cider production. &lt;br&gt;
 Disasters place a substantial burden on the health care workforce; as such, it is important to understand whether members of the health care workforce might be willing to work during disasters. The aim of this study is to explore the willingness of health care students in nursing, emergency and disaster management (EDM) departments to work during disasters and to examine sociodemographic and disaster-related factors that affect their willingness to work during disasters. &lt;/p&gt;

&lt;p&gt;A cross-sectional survey was conducted among 839 students enrolled in a Turkish University Faculty of Health Science (619 nursing students and 220 EDM students). Chi-square tests and binary logistic regressions were used to determine predictors of willingness to respond to disasters. &lt;/p&gt;

&lt;p&gt;The majority of students (62.2%) were willing to work during disasters; however, approximately 31.2% of students were uncertain, and 6.7% were unwilling to work during disasters. Approximately 85.0% of EDM students and 54.0% of nursing students were willing to work during disasters. Overall, participants were more willing to work during earthquake (71.1%) and traffic accident disasters (66.2%) but were less willing to work during contagious disease (35.1%) and gas leak disasters (33.5%). Students' willingness to work during disasters was predicted by program type, educational level and membership in a disaster-related nongovernmental organization. &lt;/p&gt;

 Antimicrobial resistance among anaerobic bacteria is increasing, leading to a growing demand for inexpensive and reliable susceptibility testing methods. The aim of this study was to determine the suitability of Fastidious Anaerobe Agar (FAA) as a medium for disk diffusion for rapidly growing anaerobic bacteria. &lt;/p&gt;

&lt;p&gt;Reproducibility of zone diameters and quality of growth were tested using six quality control (QC) strains. We compared four anaerobic incubation systems, two incubation temperatures (35°C and 37°C), and FAA from four manufacturers. The effect of incubation for 16-20hours instead of 24hours was tested on ten randomly selected isolates of the Bacteroides fragilis group. The final method was tested on 170 clinical B.fragilis-group isolates and compared to agar dilution MICs. &lt;/p&gt;

&lt;p&gt;After 24hours' incubation, all QC strains demonstrated confluent growth. The different anaerobic incubation systems were equal regarding quality of growth and zone diameters. Incubation at 35°C resulted in slightly larger zones (1-2 mm) than at 37°C. Except for Acumedia FAA, the different manufacturers showed good agreement in zone diameters. All B.fragilis-group isolates displayed confluent growth after 16-20hours. Metronidazole inhibition zones correlated well with the reference MICs. There was an area of poorer separation for meropenem and piperacillin-tazobactam between 19-27 and 14-23 mm respectively. Prolonged incubation (40-44h) of clindamycin resulted in better separation and the area of overlap was reduced from 13 to 8 mm compared with 16-20hours' incubation. &lt;/p&gt;

 People living with HIV (PLWH) are at increased risk of infections with resistant organisms due to more frequent healthcare utilization. Our objective was to investigate the association between HIV and antimicrobial resistance (AMR). &lt;/p&gt;
