      <title>Most recurring operational errors happen in simple calculations</title>

      <description>&lt;p&gt;Professionals tend to expect mistakes during complex procedures.&lt;/p&gt;

&lt;p&gt;However, recurring deviations often originate elsewhere: short mental calculations performed before executing an action.&lt;/p&gt;

&lt;p&gt;Examples:&lt;/p&gt;

&lt;ul&gt;
&lt;li&gt;checking if a value seems correct&lt;/li&gt;
&lt;li&gt;adjusting a parameter quickly&lt;/li&gt;
&lt;li&gt;validating a margin mentally&lt;/li&gt;
&lt;/ul&gt;

  The critical interval
&lt;/h2&gt;

&lt;p&gt;Between approximately 3 and 15 seconds of mental calculation:&lt;/p&gt;

&lt;p&gt;confidence remains high&lt;br&gt;&lt;br&gt;
verification decreases&lt;br&gt;&lt;br&gt;
precision becomes insufficient&lt;/p&gt;

&lt;p&gt;This can be described as a non-audited familiar calculation zone.&lt;/p&gt;

&lt;p&gt;Complex tasks trigger checking behavior.&lt;/p&gt;

&lt;p&gt;Trivial tasks trigger execution behavior.&lt;/p&gt;

&lt;p&gt;Because of that, small operational deviations accumulate not from difficulty — but from familiarity.&lt;/p&gt;
