Alexey I think it's hard but not impossible in an outsourcing company. I've been working in a software house, and I've been following those rules. I'm not saying that it was easy ;) It's tightly coupled with the client's attitude and maturity. It was my duty as a software engineer, to show our client the benefits of "no blame policy" and convince him/her to this approach.
