A failure that was, conveniently, somebody else's fault.
My old answer went something like: the deadline was unrealistic, requirements changed twice, and the release slipped. All true. Also a description of a situation rather than a decision, and every interviewer could hear that I had not taken anything personally out of it.
This question is not looking for a flaw. It is looking for evidence that you can hold a decision that went wrong.
The three part structure

Writing the story was easy. Saying it without either minimising or over apologising took repetition.
