Circumcision: From Ancient Egypt to Modern Medicine
Circumcision is one of humanity's oldest documented surgical practices. Today, it is simultaneously a religious tradition, a cultural practice, and a subject of medical debate. But how did humans arrive at the practice in the first place? Why did ancient Egyptians perform it? Why did the Israelites make it central to their religious identity? And what does 21st-century medicine actually say about it?
The answers are more complicated—and more interesting—than simply saying that circumcision is either "good" or "bad."
Ancient Egypt: Older Than Judaism
Circumcision appears to predate Judaism by many centuries.
One of the most famous pieces of evidence comes from the tomb of Ankhmahor at Saqqara, dating to approximately 2300 BCE. A relief depicts what appears to be a circumcision procedure.
This creates an important historical question:
If circumcision existed before Judaism, where did the practice originate?
We don't know.
There is no surviving ancient document telling us who performed the first circumcision or why the very first person decided to do it.
Several explanations are possible.
Hygiene and observation
Ancient societies didn't understand bacteria, viruses, or modern anatomy. But they could observe patterns.
People could notice that certain practices seemed to correlate with cleanliness, infections, wounds, or other physical outcomes without understanding the underlying biological mechanisms.
Human beings discovered countless useful medical practices this way. They didn't need germ theory to notice that some behaviors produced different outcomes.
Circumcision could have developed through a similar process.
Ritual and initiation
Another possibility is that circumcision began primarily as a rite of passage.
A painful or permanent alteration of the body can serve as a powerful symbol:
Childhood → ritual → adulthood → membership in the community.
The procedure may therefore have originally had more to do with identity and social status than medicine.
Religious or symbolic meaning
Circumcision could also have been connected to religious ideas about purity, sacrifice, sexuality, or dedication to a deity.
It is entirely possible that several of these explanations were involved simultaneously.
The Israelites and the Covenant
Judaism transformed circumcision into something much more specific.
In Genesis 17, God commands Abraham to circumcise himself, his household, and his descendants. Circumcision becomes the physical sign of the covenant between God and Abraham and his descendants.
This is a major conceptual shift.
Instead of merely:
"Our society practices circumcision."
the religious meaning becomes:
"This physical mark identifies us as participants in God's covenant."
Circumcision eventually becomes one of the defining practices of Jewish identity.
The traditional Jewish ceremony is called brit milah, meaning "covenant of circumcision."
Traditionally, it is performed on the eighth day after birth.
Did the Jews Invent Circumcision?
Probably not.
The archaeological evidence indicates that circumcision existed elsewhere before the emergence of Israelite Judaism.
The Israelites lived in the ancient Near East, surrounded by cultures that practiced various forms of circumcision. Cultural transmission is therefore one possible explanation for how the practice existed in the region.
The Israelites may have inherited or adapted an existing practice and given it an entirely new theological meaning.
The progression may have looked something like:
Ancient regional practice
↓
Israelite adoption
↓
Covenant with Abraham
↓
Religious obligation
↓
Jewish identity
This does not prove exactly how the practice entered Israelite culture. It is simply one plausible historical model.
How Could Ancient People Know It Was "Good"?
This is perhaps the most interesting question.
Ancient people didn't possess modern medical science.
They didn't know about:
- bacteria
- viruses
- HIV
- HPV
- immune cells
- DNA
- microbiomes
Yet they could still discover that certain practices had particular consequences.
Medicine existed long before modern medical theory.
A civilization could observe:
"When we do this, certain problems seem to happen less often."
The observation could then become tradition.
Eventually, the original explanation might disappear while the practice survives.
This happens throughout human history.
A behavior can survive for thousands of years because the behavior is remembered even when the original reason is forgotten.
What Does 21st-Century Medicine Say?
Modern medicine approaches circumcision very differently from ancient religion.
Medicine does not ask:
"Is circumcision sacred?"
It asks:
"What measurable effects does circumcision have on health?"
Research has identified several potential benefits.
Urinary tract infections
Circumcision is associated with a reduced risk of urinary tract infections, particularly during infancy.
HIV
One of the strongest pieces of evidence concerns heterosexual HIV transmission.
Randomized trials in several African populations found approximately a 50–60% reduction in the risk of heterosexual HIV acquisition among circumcised men.
This does not mean circumcision prevents HIV completely.
It doesn't.
It is one possible component of HIV prevention.
HPV
Circumcision is associated with lower rates of certain HPV infections.
This matters because persistent infection with high-risk HPV can contribute to several cancers.
Genital herpes
Research also indicates a reduced risk of acquiring genital herpes in certain populations.
Phimosis and balanitis
Circumcision essentially eliminates problems involving a foreskin that cannot retract normally, such as pathological phimosis.
It can also reduce recurrent inflammation of the glans and foreskin.
Penile cancer
Penile cancer is extremely rare, but circumcision is associated with a reduced risk.
What Does the Foreskin Actually Do?
This is where modern biology becomes especially interesting.
The foreskin isn't simply useless tissue.
It is normal anatomical tissue containing:
- skin
- mucosal tissue
- blood vessels
- nerves
- sensory structures
It protects the glans and allows movement of the penile skin during sexual activity.
The inner foreskin is less keratinized than ordinary external skin and contains immune cells.
That becomes relevant when discussing infections.
Why Can Circumcision Reduce HIV Risk?
The foreskin contains immune cells, including Langerhans cells and CD4+ T cells, that can participate in HIV infection.
The inner foreskin is also relatively thin and less keratinized than external penile skin.
This creates one potential route through which HIV can interact with susceptible cells.
Circumcision removes much of this particular tissue.
In simplified terms:
Intact penis
External skin → foreskin → inner mucosal surface → glans
Circumcised penis
External penile skin → glans
Circumcision therefore changes the physical environment in which certain pathogens encounter susceptible tissue.
It doesn't make the penis immune to infection.
It removes or reduces one potential route of transmission.
The Microbiome Changes Too
The area underneath the foreskin is warm and moist.
That environment supports a different collection of microorganisms than the exposed penis.
Research has found that circumcision changes the penile microbiome and can reduce certain anaerobic bacteria associated with inflammation.
That may be one mechanism contributing to reduced HIV susceptibility.
In other words, circumcision can produce two related changes:
Less susceptible tissue + a different microbial environment.
What About Micro-Injuries?
Sexual activity can produce tiny injuries to genital tissue.
Any disruption of the skin or mucosa can potentially make it easier for pathogens to reach underlying tissue.
The foreskin and frenulum can experience such injuries.
Removing much of that tissue may therefore reduce certain opportunities for pathogens to enter.
Again, this doesn't make circumcision a complete barrier against infection.
So Is an Uncircumcised Penis Unhealthy?
No.
This distinction is extremely important.
An intact foreskin is normal human anatomy.
An uncircumcised man can maintain excellent hygiene, have normal sexual function, and remain completely healthy.
Circumcision changes the statistical probability of certain medical outcomes; it does not divide men into "healthy" and "unhealthy" categories.
Religion and Medicine Are Making Different Arguments
This distinction is perhaps the most important conclusion.
Ancient Judaism says:
Circumcision is a sign of the covenant.
Modern medicine says:
Circumcision changes the probability of certain health outcomes.
Those are fundamentally different claims.
Science can investigate whether circumcision changes HIV transmission, HPV infection, UTIs, or other medical outcomes.
Science cannot establish whether God commanded Abraham to circumcise himself.
That is a theological question rather than an empirical medical one.
The Mystery of the First Circumcision
Ultimately, one question remains unanswered:
Who was the first person to decide to circumcise another person?
We don't know.
The practice is ancient enough that its origins disappear into prehistory.
Perhaps it began as medicine.
Perhaps ritual.
Perhaps initiation.
Perhaps religion.
Perhaps hygiene.
Perhaps social identity.
Perhaps several of these simultaneously.
What we do know is that circumcision became extraordinarily important in different civilizations for very different reasons.
Ancient Egypt practiced it.
Ancient Israel transformed it into a covenantal sign.
Judaism preserved it for thousands of years.
And modern medicine eventually discovered that the procedure has measurable biological effects.
That makes circumcision an unusual intersection of biology, religion, culture, history, identity, and medicine.
The ancient Egyptians didn't need to understand HIV.
The ancient Israelites didn't need to understand immune cells.
And modern scientists don't need to invoke theology to measure the consequences.
Each period is answering a different question.
Ancient religion asked: "What does this mark mean?"
Ancient societies asked: "Why do we do this?"
Modern medicine asks: "What does this physically do?"
And humanity is still trying to understand how all three questions became attached to the same piece of anatomy.
