PTSD is rated under the General Rating Formula for Mental Disorders (38 CFR §4.130). Unlike musculoskeletal conditions, it's rated on occupational and social impairment — not test results.

Symptoms that support 30%:
- Depressed mood
- Anxiety
- Panic attacks more than once a week
- Chronic sleep impairment
- Mild memory loss (names, directions, events)
50% ($1,075/month):
"Occupational and social impairment with reduced reliability and productivity..."
Symptoms that support 50%:
- Flattened affect (reduced emotional expression — hard to feel happy, flat tone)

- Disturbances of motivation and mood

The 30% vs. 50% distinction in plain language:
At 30%, the CFR sees someone "generally functioning satisfactorily." At 50%, it sees someone whose reliability and productivity are reduced — not just occasionally affected. If your PTSD symptoms cause you to miss work, have conflict with supervisors, avoid coworkers, or struggle to complete complex tasks consistently, that's 50% language, not 30%.

The Stressor Statement Requirement
Unlike most VA conditions, PTSD claims require a stressor statement — a written account of the in-service event that caused the PTSD. Without a specific, qualifying stressor documented in your claim, VA cannot establish service connection regardless of your diagnosis.
A stressor qualifies if:
- You personally experienced or witnessed actual or threatened death, serious injury, or sexual violence
- You learned about the actual or threatened violent or accidental death of a family member or close friend
- You experienced repeated or extreme indirect exposure to aversive details of qualifying events
Generic language ("I had combat stress" or "my job was stressful") does not satisfy the stressor requirement. The stressor statement must be specific: unit, approximate date, location, what you witnessed or experienced.
PACT Act and PTSD
The PACT Act expanded presumptive service connection for many toxic exposure conditions.
