VA PTSD Rating Criteria: The 30% vs 50% vs 70% Distinction That Costs $500/Month

The difference between a 30% PTSD rating and a 50% rating is approximately $550/month. Most veterans at 30% who describe their symptoms clearly would qualify for 50%. Here are the exact criteria from 38 CFR §4.130 (DC 9411).

30% — Occasional Decrease in Work Efficiency

  • Depressed mood, anxiety, suspiciousness
  • Panic attacks more than once a week
  • Chronic sleep impairment
  • Mild memory loss (forgetting names, directions, recent events)
  • Disturbances of motivation and mood

50% — Reduced Reliability and Productivity

  • Flattened affect
  • Difficulty understanding complex commands
  • Impairment of short- and long-term memory (retention of only highly learned material)
  • Difficulty adapting to stressful circumstances
  • Impaired abstract thinking
  • Difficulty establishing and maintaining effective work and social relationships

70% — Occupational and Social Impairment in Most Areas

  • Suicidal ideation
  • Near-continuous panic or depression
  • Impaired impulse control (unprovoked irritability with periods of violence)
  • Spatial disorientation
  • Neglect of personal appearance and hygiene
  • Inability to establish and maintain effective relationships

100% — Total Occupational and Social Impairment

Persistent delusions or hallucinations, disorientation to time or place, memory loss for names of close relatives, inability to perform activities of daily living.

Know your tier language: Use the regulatory language when describing symptoms to the examiner.

MST alternative evidence: Claims can use alternative evidence under 38 CFR §3.304(f)(5) — personal statements, behavioral changes documented in service records.
