I have had such an experience in 2020. We hired a tester to join a team only made of developers. We told our manager that simply hiring someone wouldn't be enough, that we would need to change our process, write down specifications, etc. Management understood but the actions were insufficient. Let's mention that it was during lockdown (thx Covid19). Of course, it didn't work at all.
The tester left the company for personal reasons, not related to the project. Now the software is being tested by an external company. Things are working correctly because we have learnt from the previous experience.
