Seriously, don't give up your references until you're at the offer stage (which has been industry standard reference check etiquette for decades). When staffing agencies won't respect your references' time or acknowledge proper reference check etiquette, you have to protect the people you depend upon.
And there's no excuse to make a reference fill out some excessive form (in some cases, it may even be illegal to require some of the details I've seen).
