Here's the pattern: the build is green, the demo works, and the engineer's map of the system is still wrong.
Tests only check the paths we wrote down, so a bad assumption about state, timing, ownership, or failure can sit there quiet until prod brings real pressure. That gap is where rework, drift, and fake confidence enter the system bc the model was wrong before the code was wrong.
That is why I treat quality as operating evidence, not a final inspection. I measure whether the engineer can explain the change, name edge cases, read the telemetry, and repair the work when the model breaks. AI can make syntax clean fast, but it cannot own the system or carry the pager when the answer was wrong.
TeamStation's quality doctrine gets into the math behind that problem: does the mental model match the real system under pressure? For distributed LATAM teams, distance is just another variable when review rails, telemetry, and feedback are explicit.
