Most code reviews do not fail because reviewers are lazy.
They fail because reviewers are forced to spend attention on the wrong things.

- documentation: the checklist becomes a visible artifact of what was verified

A tiny workflow that works
Here is a review loop that is simple enough to survive contact with a real team.
1) Gate the PR with automation
Do not ask for review until mechanical checks are done.
That means CI should answer questions like:
- does it build?
- do the relevant tests pass?
- does it violate obvious style or type rules?
If those fail, the author fixes them before involving another person.
2) Add a visible checklist to the PR description

Small automations that pay for themselves
You do not need a massive internal platform to get value here.
A few targeted automations usually cover most of the pain.
Run only affected tests
If a change touches a single package, do not run the whole universe.
Fast, relevant feedback keeps authors honest and reviewers patient.
Post a compact CI summary
Instead of making reviewers dig through logs, post a short status summary:

That sounds trivial, but it removes friction immediately.
Auto-surface simple fixes
Formatting and low-risk static analysis comments should show up as inline suggestions or bot annotations, not as reviewer prose.
Humans should spend their comments on reasoning, not commas.

A tiny script that turns PR metadata into a short changelog blurb helps reviewers and release managers at the same time.
It also pushes authors toward clearer PR titles.

- “What happens if limit is negative?”
Only the last question is high-value review work.
With a checklist-driven flow, the first three should already be solved before the reviewer arrives.
Then the conversation can focus on the actual risk:
- what happens with invalid limits?
- is this backward compatible?
- do we need a cap?
- are there tests for empty and large datasets?
That is what you want.
Keep automation from becoming noise
Automation helps until it starts shouting.
A few rules keep it useful:
- do not run expensive checks on every tiny change if they are not needed
- suppress flaky or non-actionable noise
- keep bot comments short and link to logs instead of dumping them inline
- review the checklist itself every few months and remove dead items
If nobody trusts the signals, people will ignore them.
Use the checklist as a teaching tool
One underrated benefit: checklists make review quality easier to teach.
When the same review failure shows up repeatedly, you can ask:
- should this become a pre-commit hook?
- should CI enforce it?
- should the checklist be clearer?
- do we need a short style guide example?
That is how teams slowly move recurring mistakes out of human review and into automation.
