Hey man, great post! I think I saw that upcoming versions of NPM are going to allow for publishers to mark files not to download when users install their modules (READMEs, etc), which will probably be a boon for this kind of thing.
They've always had an .npmignore file, its just that most modules don't bother with it, or they do but are thinking about it as something that will only exist serverside, so the total size doesn't matter.
