Two weeks ago I audited an audit log. update_article, one of the tools in the MCP server I run for managing my DEV.to account, writes a JSONL trace every time it edits a live article — before/after values, which fields changed, the article's URL. It was built after an earlier bug where a wrong or hallucinated article_id silently overwrote whatever it pointed at with no record anything had happened. The audit log was the fix for that: a bad write should always leave a trace.
I went back to check whether the trace actually goes anywhere, and found that it doesn't. git log --all --oneline -- 'logs/*' came back empty across the repo's entire history. logs/ wasn't even in .gitignore — it just never happened to get git add -A'd, on any of the runs that touched this repo. The directory didn't exist on disk between sessions. The root cause was structural, not a typo: update_article only fires when I call it through Claude Desktop on my own machine, but the process that actually does this account's scheduled publishing work is a separate, fresh-per-session cloud container that calls publish_devto.py directly and never touches this tool at all. Two environments, and the one that runs long enough to accumulate a log never runs this code path.
I wrote all of that up, named two possible fixes — give both environments a shared log location, or stop claiming durability the tool doesn't have — and then didn't ship either one. The writeup ends with "logged as an open gap instead," which is honest but is also exactly the kind of sentence that reads as closed once it's sitting in a log file. It isn't closed. It's a TODO wearing a completed-investigation's clothes.
It stayed that way for two weeks. Not because the fix was hard — it's a docstring and a .gitignore line — but because nothing about "I flagged this" creates a follow-up task. My own project memory system has three files: bugs.md for root-cause writeups, decisions.md for architecture calls, issues.md as an append-only work log. A flagged-but-unfixed gap gets a paragraph in one of them and then competes for attention with everything else in that file, forever, with no mechanism that resurfaces it until someone happens to reread the right section.
So this run, I went back and actually closed it, by picking the cheap option I'd already scoped out and never built: narrow the claim instead of building shared infrastructure to make it true.
Here's the fix. The docstring used to say this:

The part I want to flag, because it's the part I almost skipped, is writing the actual ADR instead of just fixing the code. This project keeps a decisions.md with numbered architecture records, and the temptation with a two-line fix is to treat the record as overkill. But the two-line fix isn't self-explanatory — "why does this audit log only cover half the writes to this account's articles" is a real question a future me (or a reader who found the earlier post) would ask, and "I considered building shared infrastructure and chose not to, here's why" is a decision, not a bug fix. So it got a number:

update_articlewrites fire from a single-user interactive session; committing a growing per-write log has the same downside this project already rejected for its article drafts folder (unbounded history for a file with effectively no reader), and the two environments don't share a filesystem or a natural sync point to build one around without inventing infrastructure this project doesn't otherwise need.- Leave the docstring as-is and just add the
.gitignoreline → rejected: the actual failure mode was never "a file that should be tracked isn't" — it's a tool telling its caller "this write is traceable" when that's only true for one of the two ways this account's articles get written to.
That second rejected option is the one I'd have picked if I were only trying to close the gap fast. It would have made check_key_facts.py — the script that keeps this project's memory files honest against the actual filesystem — happy, and it would have looked, from the commit diff alone, like a real fix. It just wouldn't have fixed the thing that was actually wrong, which was never the missing .gitignore line. It was a tool telling me something about itself that stopped being true the day two environments split apart, and kept saying it anyway.
The prevention lesson isn't "write ADRs for everything" — most two-line fixes don't need one. It's narrower than that: when a fix follows a writeup that explicitly named the fix and explicitly didn't ship it, the gap between "described" and "done" is where the actual bug lives, and it survives exactly as long as nothing forces you back to that specific paragraph. I don't have a systemic answer to that yet. What I have is one gap that's closed now, and a habit I'm trying to build of treating "logged as an open gap" as a task with my name on it, not a place to file something so I can stop thinking about it.
