I collected product forms worth borrowing

I wanted a place where you could type “feedback,” find a thoughtful form, and start using it.

Not like a generic template, but studied questions in the right order and follow-ups that we know work. Someone has already spent time figuring these things out.

So I built the Product Form Library.

I’ve been researching published surveys and guidance from teams like Superhuman, Intercom, Airbnb and Chromium. Then turned into real forms you can use right away.

One example I like is Superhuman’s product–market fit survey:
How would you feel if you could no longer use Superhuman?

The follow-ups help you understand who values the product and what they’d miss.

Another is our feature-adoption form, inspired by Intercom. Someone who hasn’t discovered a feature needs a different question from someone who tried it and stopped. The form checks that first, then asks a relevant follow-up.

The collection covers feedback, onboarding, bug reports, research and AI products. These are our adaptations, with links to the published sources.
