Why PDFs End Up Rotated Wrong

Mobile scanning apps — Phone camera scanning apps occasionally misread page orientation, especially for A4 pages scanned in portrait mode.
PDF merging — When combining PDFs from different sources, some pages may have been created in landscape while others are portrait. Merging preserves each page's original rotation.
Printer settings — PDFs generated from print dialogs can inherit the wrong page orientation if the printer driver settings don't match the document layout.
