Part 1: A three-line recap (proves you listened)
Not a transcript. Three lines that show you understood the point.
"Quick recap: the priority is getting the reporting dashboard live before the board meeting on the 28th. Design is flexible; the data accuracy is not. Budget is set, timeline is the constraint."
If you can compress a 45-minute call into three accurate lines, the client trusts you with the whole thing.
Part 2: An action-item table (removes ambiguity)
A tiny table with three columns: what, who, by when. Include their items too — this is the part that makes you look like a project lead, not a vendor.
