Start Here: Agent Operations That Survive Production

I run approval-gated agent operations for a living — batches of subagents doing real work, with a human who has to be able to trust the results. These pages are my operating record: the patterns that survived production, written up as they happened.

The three patterns so far

1. The Dive — the verification step your agent batch is missing. A coordinator says "done"; a diver re-checks the claim against the evidence before anyone acts on it. Read it: https://dev.to/veronicstewart/the-dive-the-verification-step-your-agent-batch-is-missing-4e8i — Watch it (60 seconds): https://www.youtube.com/shorts/YnqrBoIpgKs

2. Approval fatigue — the rubber stamp is worse than no lock at all. When approvals become routine, the lock stops protecting anything. Read it: https://dev.to/veronicstewart/approval-fatigue-the-rubber-stamp-is-worse-than-no-lock-at-all-ale — Watch it: https://www.youtube.com/shorts/Q44kfY10I90

3. Standing authority — "Just go do it" — and the system didn't care. A standing grant from a human means nothing if the runtime hard-locks the action anyway; batch approvals are the working compromise. Follow the record

If one of these patterns saves your batch, repost it for the next operator. That's how this record travels.
