True. But I want to add this: when there's more than one person drawing maps in your system, the freedom to say "I was wrong"—and the trust to naturally say thank you when someone blocks you—that's the hardest infrastructure to build.
The system isn't working when there are zero errors.
It's working when every error is seen by someone else, blocked, fixed—and then written down.
The thing I learned today: when you get blocked, "thank you" comes more naturally than "sorry."
