The Usability Professionals Association made a chutes-and-ladders style poster for the UX process (google "chutes and ladders UX"). It covers most everything - and each subject can be googled individually for more in-depth analysis. I'd say it misses out on mentioning Information Architecture, uses "user profiles" instead of "personas", and maybe a couple other gotchas.
