The Usability Professionals Association made a chutes-and-ladders style poster for the UX process (google "chutes and ladders UX"). It covers most everything - and each subject can be googled individually for more in-depth analysis. I'd say it misses out on mentioning Information Architecture, uses "user profiles" instead of "personas", and maybe a couple other gotchas.

Two books I cannot recommend more highly:

"Don't Make Me Think" by Steve Krug sensible.com/dmmt.html

"Design For Real Life" by Sara Wachter-Boettcher and Eric Meyer abookapart.com/products/design-for...
