I'm considering applying for a remote job at a company based in Toronto (I live in West Virginia, USA). However, it looks like Toronto salaries are a lot lower than anywhere in the US. Should I ask up front about the salary range, so I don't end up going through their whole interview process only to get a lowball offer?
Would a Canadian company hiring US workers usually offer health insurance?
Should I expect to negotiate in Canadian dollars? Would my salary in USD fluctuate as currencies shift in relative price?

Here's what I'm thinking: I send in my Resume and cover letter, and if they show interest, at that point I say something like "I hate to ask about salary this early in the process, but since I don't know what to expect from international work, I was wondering if you could tell me the salary range of the position, or at least whether you set your payscale according to the location of the employee or some other factor?"
I don't want to sound like I only care about money. I do care about money, but I also care about finding a good company where I can do good work.
