Claude Code loads your CLAUDE.md, the name and description of every skill, and the tool schemas of every MCP server into context at the start of each session. That is a fixed cost on every conversation, and it is easy to forget about.
I wrote a small zero-dependency Python script that estimates it (characters divided by four, so treat it as a rough guide).

What I found on my own setup
With 55 skills installed, preloaded metadata cost about 11,600 tokens per session, while the bodies together are far larger but only load when a skill is invoked. One skill alone has a body of 64k tokens. That is fine, because it only loads on demand. The lesson: put procedures in skills, keep CLAUDE.md short, and prune skills and MCP servers you do not use.
